مقایسه دو روش درمانی اکسترنال فیکساسیون با فیکساتور خارجی توبولار AO و میله داخل کانال استخوانی بدون ریم در انواع شکستگی های باز 3/1 میانی تیبیا نوع IIIA و IIIB

Abstract

In this study, we aimed to compare the outcome and consequences of OA tubular external fixation vs. unreamed intramedullary nailing in open grade IIIA-IIIB tibial shaft fractures. In a randomized clinical trial, 50 patients with open tibial shaft fractures (grades IIIA-IIIB) were recruited in Tabriz Shohada teaching centre in a 27-month period of time. They randomized in two equal groups underwent either AO tubular external fixation or unreamed intramedullary nailing. These 2 groups were matched for sex, age and fracture-grade. The follow-up time was one year. Union time, surgical outcome, postoperative complications and the ambulation time were compared between the two groups. Twenty five patients, 20 males and 5 females with a mean age of 30.805.24 years were allocated in unreamed intramedullary group and 25 other patients, 22 males and 3 females with a mean age of 28.928.88 years were studied in the external fixation group (p=0.70 and 0.37, respectively). The time of union was 3, 4, 5 and 6 weeks after operation in 28%, 12%, 32%, and 28% of the cases in unreamed intramedullary group vs. 4%, 12%, 48%, and 36% of the cases in external fixation group, respectively (p=0.14). Post-operative infection, soft tissue injury, malunion and nonunion were documented in 16%, 8%, 0% and 4% of the cases in unreamed intramedullary group vs. 32%, 12%, 24%, and 8% of the cases in external fixation group, respectively (p=0.19, 0.50, 0.02 and 0.50, respectively). The mean ambulation time after operation was 2.922.43 weeks in the unreamed intramedullary nailing group vs. 2.682.14 weeks in the external fixation group (p=0.71)., هدف از این مطالعه، مقایسه سرانجام و نتایج فیکساسیون خارجی توبولار AO و استفاده از میله اینترامدولاری بدون ریم در درمان شکستگی های تنه تی بیا درجه IIIA-IIIB می باشد.در یک کارآزمایی بالینی تصادفی شده، 50 بیمار با شکستگی های باز تنه تی بیا (درجه IIIA-IIIB) طی مدت 27 ماه در مرکز آموزشی-درمانی شهدا تبریز بررسی شدند. این بیماران بطور تصادفی به دو گروه مساوی تقسیم و تحت درمان با فیکساسیون خارجی توبولار AO یا استفاده از میله اینترامدولاری بدون ریم قرار گرفتند. این دو گروه از نظر جنس، سن و درجه شکستگی همسان بودند. مدت پیگیری یک سال بود. زمان جوش خوردگی، نتیجه جراحی، عوارض پس از عمل و زمان حرکت بین دو گروه مقایسه شد.بیست و پنج بیمار، 20 مذکر و 5 مونث با سن متوسط 24/580/30 سال در گروه اینترامدولاری بدون ریم و 25 بیمار دیگر، 22 مذکر و 3 مونث با سن متوسط 88/892/28 سال در گروه اکسترنال فیکساسیون بررسی شدند (p بترتیب 70/0 و 37/0). زمان جوش خوردگی در 28%، 12%، 32% و 28% بیماران گروه میله اینترامدولاری بدون ریم بترتیب 3، 4، 5 و 6 هفته و در گروه اککترنال فییکاسیین بترتیی 4%، 12%، 48% و 38% بود (14/0=p). عفونت پس از عمل، آسیی بافت نرم، بدجوش خوردگییو عدم جوش خوردگییدر گروه مییه اییترامدولارییبدون ریی بترتیی 16%، 8%، 0% و 4% و در گروه اککترنال فییکاسیین بترتیی 32%، 12%، 24% و 8% بود (p بترتب 19/0، 50/0، 02/0 و 50/0). متوسط زمان راه افتادن پس از عمل در گروه مییه اییترامدولارییبدون ریی 43/292/2 هفته و در گروه اککترنال فییکاسیین 14/268/2 هفته بود (71/0=p).
