بررسی الکترودیاگنوستیک سیستم اعصاب محیطی در بیماران مبتلا به مولتیپل اسکلروز

Abstract

The purpose of this study was electro diagnostic peripheral nervous system evaluation in patients with multiple sclerosis. In a cross sectional and descriptive analytical study was conducted on patients with MS in the Department of Neurology University of Medical Sciences, changes in NCS in MS patients we examined. 19 of studied patients were male and 56 of them were female. MS disease in 20 patients (26.7%) patients was SP and in 55 (73.3%) patients were RR. The mean age of patients with type SP was 41.70 11.93 and the mean age of patients with type RR, was 30.23 9.51years (P<0.001). Mean involvement duration in patients with type SP was 8.85 5 and the mean involvement duration in patients with type RR was 2.49 2.83 years (P<0.001). Mean EDSS in patients with type SP was 6.05 0.85 and the mean EDSS in patients with type RR was 2.92 1.44(P<0.001). Most amplitude proneal motor disorder occurred in 22.66% and 11.33% in the sural amplitude disturbance. Maximum Latency occurred in tibial motor disorder in 16.66% of cases and the most disorder in the median and peroneal motor Velocity occurred at 9.33% of cases there was also the most impaired in F_Wave peroneal H_Reflex disorder occurred in 24.66% and 9.33%. In our study, median nerve motor amplitude of left tibial and peroneal motor was significant less than the normal mean, and Latency of right median nerve motor as well as significant less than mean normal society. Relationship between electro diagnostic findings in patients with age, disease duration and relapse number of disease and spinal cord involvement, the number of brain plaques, EDSS of patients studied did not exist. Mean Velocity motor nerves studied no significant differences with middle mean. Mean Amplitude of median motor left; mean Amplitude of tibial and peroneal motor was significant amount less than normal., هدف از این مطالعه بررسی الکترودیاگنوستیک سیستم اعصاب محیطی در بیماران مبتلا به مولتیپل اسکلروز بود.در یک مطالعه مقطعی توصیفی تحلیلی که بر روی بیماران مبتلا به MS در دپارتمان نورولوژی دانشگاه علوم پزشکی تبریز انجام گرفت، تغییرات NCS در بیماران مبتلا مولتیپل اسکلروز را مورد بررسی قرار دادیم.19 نفر از بیماران مورد مطالعه مرد و 56 نفر زن بودند.نوع بیماری MS در 20 نفر(7/26%) از بیماران SP و در 55 نفر(3/73%) از بیماران RR بود.میانگین سن بیماران مبتلا به نوع SP، 93/11 70/41 و میانگین سن بیماران مبتلا به نوع RR، 51/9 23/30 سال بود(001/0P<). میانگین مدت ابتلا بیماران مبتلا به نوع SP، 00/5 85/8 و میانگین مدت ابتلا بیماران مبتلا به نوع RR، 83/2 49/2 سال بود(001/0P<).میانگین EDSS بیماران مبتلا به نوع SP، 85/0 05/6 و میانگین EDSS بیماران مبتلا به نوع RR، 44/1 92/2 بود(001/0P<). بیشترین اختلال آمپلی تود پرونئال حرکتی در 66/22% موارد و اختلال آمپلی تود سورال در 33/11% موارد وجود داشت. بیشترین اختلال Latency در تیبیال حرکتی در 66/16% موارد و بیشترین اختلال Velocity در مدین و پرونئال حرکتی در 33/9% موارد وجود داشت و همچنین بیشترین اختلال در F_Wave در پرونئال در 66/24% و اختلال H_Reflex در 33/9% موارد وجود داشت. در مطالعه ما آمپلی تود اعصاب مدین حرکتی چپ، تیبیال و پرونئال حرکتی بصورت معنی دار کمتر از میانگین نرمال جامعه بود و Latency عصب مدین حرکتی راست نیز بصورت معنی دار کمتر از میانگین نرمال جامعه بود. ارتباطی بین بین یافته های الکترودیاگنوستیک در بیماران با سن، مدت ابتلا به بیماری و تعداد عودهای بیماری و همچنین درگیری نخاعی، تعداد پلاک های مغزی،EDSS بیماران مورد مطالعه وجود نداشت. میانگین Velocity اعصاب حرکتی مورد مطالعه تفاوت معنی داری با میانگین متوسط جامعه نداشت. میانگین Amplitude مدین حرکتی سمت چپ، میانگین Amplitude تیبیال و پرونئال حرکتی بصورت معنی داری کمتر از مقدار نرمال بود.
