مقایسه آنژیو گرافی با 64 Row Multi detector Computed Tomography و آنژیوگرافی تهاجمی در تشخیص تنگی قابل اهمیت شرایین کرونری

Abstract

This study aims at comparing the 64-slice MDCT and ICA in diagnosis of significant coronary arteries stenosis. In a cross sectional study, we evaluated the diagnostic accuracy of 64-slice MDCT versus ICA in patients with the suspected CAD who underwent both MDCT and ICA (as the gold standard). Computed tomography angiography and invasive coronary angiography findings of each coronary segment were compared to determine the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of MDCT in the detection of significant lesions( 50% diameter decrease stenosis or total occlusion with no luminal flow shown). The sensitivity of 64-slice MDCT in diagnosis of significant CAD was 69.8%, 86.3% and 97.6% in segment, vessel and patient-based analysis, respectively. The specificity of 64-slice MDCT was 94.8%, 92.2% and 85.2% in segment, vessel and patient-based analysis, respectively. The PPV of 64-slice MDCT was 69%, 86.6% and 95.2% in segment, vessel and patient-based analysis, respectively. The NPV of 64-slice MDCT was 95%, 92.2% and 92% in segment, vessel and patient-based analysis, respectively. The accuracy of 64-slice MDCT was 91.3%, 90.3% and 94.5% in segment, vessel and patient-based analysis, respectively., هدف از این مطالعه مقایسه آنژیوگرافی با 64-slice MDCT و آنژیوگرافی تهاجمی در تشخیص تنگی قابل اهمیت شرایین کرونری می باشد. در یک مطالعه مقطعی، ما دقت تشخیصی 64-slice MDCT را در برابر ICA در بیماران مشکوک به CAD که تحت هر دو روش MDCT و ICA (بعنوان استاندارد طلایی) قرار گرفته بودند، بررسی نمودیم. یافته های آنژیوگرافی با استفاده از توموگرافی کامپیوتری و آنژیوگرافی تهاجمی در هر سگمان مقایسه گردید تا حساسیت، ویژگی، ارزش اخباری مثبت و ارزش اخباری منفی MDCT در تشخیص تنگی قابل توجه (تنگی 50% قطر رگ یا انسداد کامل جریان) تعیین شود.حساسیت 64-slice MDCT در تشخیص تنگی قابل اهمیت شرایین کرونری در مقایسه با آنژیوگرافی تهاجمی کرونر، در سگمان، در شریان و در بیمار بترتیب 8/69%، 3/86% و 6/97% بود. ویژگی 64-slice MDCT در سگمان، در شریان و در بیمار بترتیب 8/94%، 2/92% و 2/85% بود. ارزش اخباری مثبت 64-slice MDCT در سگمان، در شریان و در بیمار بترتیب 69%، 8/86% و 2/95% بود.ارزش اخباری منفی 64-slice MDCT در سگمان، در شریان و در بیمار بترتیب 95%، 2/92% و 92% بود. دقت 64-slice MDCT در سگمان، در شریان و در بیمار بترتیب 3/91%، 3/90% و 5/94% بود.
