بررسی تاثیر دوگزیوم بر سطح سرمی high sensitivity CRP و Endothelin-1 در بیماران با رتینوپاتی دیابتی

Abstract

Background: Diabetic retinopathy (DR) is a highly specific vascular complication of type 1 and type 2 diabetes mellitus. Calcium dobesilate(CD) or Doxium has been tested in the treatment of diabetic retinopathy showing a slowdown of the progression of the disease after long-term oral treatment,as a potent antioxidant. Endothelin-1 (ET-1) Besides being a very potent vasoconstrictor,acts as a mitogen on the vascular smooth muscle and play the main role in the failure of autoregulation that it is an important and often early feature of diabetic retinopathy.several studies have been confirmed that inflammation besides oxidative stress are the main mechanisems,in the pathogenesis of DR and hsCRP can play a sensitive role in detecting inflammation in these patients. The aim of this study was to determine the effects of CD on decresing ET-1 and hsCRP serum levels in patients with diabetic retinopathy. Methods & Materials: Ninety adults with type II diabetes and diabetic retinopathy were included in this double-blind randomized clinical trial.We devided them randomly in two groups of case and control,age and sex-matched randomly between two groups during seventeen month period in Tabriz Nikookary Hospital and Sheikholrais clinical center. Treatment was CD 500mg daily in case group for three month,The serum level of hsCRP and ET-1, was determined in all patients before and after administration of CD and compared between the two groups. Results: Ninety patients with diabetic retinopathy,including of 30 males and 60 females, with the mean age of 59.187.13 years were enrolled in the study. The mean level of serum endothelin-1 was significantly different in the case group than control group after treatment with Doxium as well (0.690.32 pg/ml vs. 0.860.30 pg/ml ;p=0.005),and also there was a significant different of mean serum level of hsCRP after treatment with Doxium between the case and control group, as well (3.28 3.28 mg/L vs. 4.673.75 mg/L; p=0.006;respectively).There was a significant correlation between endothelin-1 and hsCRP after treatment with doxium in case group(rh=0.428,p=0.002)., این مطالعه کمک کرده است تا اثر کلسیم دوبسیلات روی عروق و رتینوپاتی دیابتی را با کاهش سطحET-1 و hsCRP در بیماران با رتینو پاتی دیابتی مشخص کند.مواد وروشها :90فردبزرگسال با دیابت تیپ II و رتینوپاتی دیابتی در این کار آزمایی بالینی تصادفی دو سویه کور وارد شدند،که به صورت تصادفی به دو گروه مورد وشاهد تقسیم شدند،سن و جنس بین دو گروه به صورت تصادفی و برابر در مدت 17 ماه در بیمارستان نیکو کاری تبریز و مرکز کلینیکی شیخ الرئیس تقسیم شد.درمان با کلسیم دوبسیلات mg 500 روزانه در گروه مورد در مدت سه ماه انجام شد و سطح سرمی ET-1 و hsCRPدر تمام بیماران قبل و بعد از تجویز کلسیم دو بسیلات مشخص و بین دو گروه مقایسه شد.نتایج :نود بیمار با رتینو پاتی دیابتی شامل 30 مرد و 60 زن با میانگین سنی 13/ 7 18/59 وارد مطالعه شدند.میزان متوسط سطح سرمی اندوتلین-1 به طور واضح و معنی داری در گروه مورد متفاوت از گروه شاهد،بعد از درمان با دو گزیوم بود(005/0=p pg/m;30/0 86 /0 در مقابلml/pg32/069/0) و همچنین تفاوت مشخص و معنی داری بین متوسط سطح سرمی hsCRP بین دو گروه مورد و شاهد بعد از درمان با دوگزیوم وجود داشت(006/0p= ; mg/l75/367/4 در مقابل 28/328/3).همچنین همبستگی معنی داری از نظر آماری بین ET-1و hsCRP بعد از درمان در گروه مورد وجود داشت(002/0 =p ; 428/0=rho ).
