کله سیستکتومی لاپاراسکوپیک با کمک SonoSurg و مقایسه با لاپاراسکوپی روتین

Abstract

This study aims at evaluating the results of laparoscopic cholecystectomy by SonoSurg and comparing the results with conventional method. In a clinical trial, 100 patient candidates for laparoscopic cholecystectomy were randomized in two 50-patient groups and operated by either the routine method or by application of SonoSurg modalities. In the routine method group, tow cases converted to open surgery. Intra and post-operation complications during hospitalization, duration of operation and hospital cost were compared between the two groups. Forty eight patients, 12 males and 36 females with the mean age of 37.708.08 years were enrolled in the routine group and 50 patients, 10 males and 40 females with the mean age of 50.629.04 years were enrolled in the SonoSurg group (p>0.05). Major intraoperational hemorrhage was not seen in any patients of the two groups. Likewise, there was no significant complication during and after operation until discharge in any patient. The mean operation duration was 59.7914.33 min and 53.0012.41 min in the routine and SonoSurg groups; respectively (p=0.014); i.e., the mean operation time was significantly shorter in the SonoSurg group. The mean total cost was comparable for routine and SonoSurg groups (1330000.50130000.36 rials vs. 1350000.28130000.01 rials, respectively; p=0.506)., هدف از این مطالعه بررسی نتایج کوله سیستکتومی لاپاروسکوپیک با استفاده از SonoSurg و مقایسه آن با روش مرسوم می باشد.در یک مطالعه از نوع کارآزمایی بالینی، 100 بیمار کاندید کوله سیستکتومی لاپاروسکوپیک بطور تصادفی به دو گروه 50 نفری تقسیم شده و تحت جراحی به روش مرسوم و یا با کمک SonoSurg قرار گرفتند. در دو بیمار از گروه مرسوم لاپاروسکوپی به لاپاروتومی تبدیل شد. عوارض حین و پس از بستری، مدت عمل جراحی و هزینه بیمارستانی در دو گروه مقایسه گردید.48 بیمار، 12 مذکر و 36 مونث با سن متوسط 08/870/37 سال در گروه مرسوم و 50 بیمار، 10 مذکر و 40 مونث با سن متوسط 04/962/50 سال در گروه SonoSurg تحت عمل قرار گرفتند (05/0<p). خونریزی عمده حین عمل جراحی در هیچ یک از بیماران دو گروه مشاهده نگردید. علاوه براین، هیچ عارضه قابل توجهی حین و پس از عمل طی دوره بستری تا ترخیص وجود نداشت. متوسط زمان جراحی در گروه مرسوم و SonoSurg بترتیب 33/1479/59 و 41/1200/53 دقیقه بود (014/0=p)؛ بعبارت دیگر متوسط مدت عمل در گروه SonoSurg بطور معنی داری کمتر بود. متوسط هزینه کلی در گروه مرسوم و SonoSurg تفاوت معنی دار آماری نداشت (بترتیب 36/13000050/1330000در برابر 01/13000028/1350000 ریال، 506/0=p).
