نقش اندوسکوپی زودرس دستگاه گوارش فوقانی در تعیین نحوه درمان سوختگی ناشی از بلع مواد سوزاننده در بالغین

Abstract

This study aims at evaluating the role of early upper gastrointestinal endoscopy in management of corrosive ingestion in adults. In an analytic-descriptive and cross-sectional study, 100 adult patients with definite diagnosis of injury due to corrosive ingestion on direct laryngoscopy were evaluated in Tabriz Imam Khomeini Hospital during a 15-month period. The patients with signs and symptoms indicating a serious internal injury were operated immediately. In other patients, upper gastrointestinal endoscopy was employed and according to timing, patients categorized in two groups; early endoscopy (24 h). Inhospital condition of the two groups was compared. One hundred patients, 62% females and 38% males with a mean age of 25.519.25 years were recruited. Corrosive ingestion was for suicidal attempt in 53% and accidental in 47% of the cases. Alkaline materials were used in majority of cases (83%) and acidic agents were seen in 17% of the patients. Eight percent of the patients were operated immediately according to presenting signs and symptoms. Early endoscopy was employed in 37% of patients and 55% underwent late endoscopy. In latter two groups, endoscopy yielded a normal finding in 11%, grade I lesion in 36%, grade II lesion in 22%, grade III lesion in 11% and grade IV lesion in 12% of the patients. There was not a significant difference between the two groups regarding the need of operation, gap between admission and operation, need of ICU admit and its duration and inhospital morbidity and mortality. However, operation gap and ICU admit were apparently better in early endoscopy group and morbidity was lower in late endoscopy group., هدف از این مطالعه، بررسی نقش اندوسکوپی زودرس دستگاه گوارش فوقانی در اداره سوختگی ناشی از بلع مواد سوزاننده در بالغین است.در یک مطالعه توصیفی-تحلیلی و مقطعی، 100 بیمار بالغ با تشخیص قطعی سوختگی ناشی از بلع مواد سوزاننده در لارنگوسکوپی مستقیم در بیمارستان امام خمینی تبریز طی مدت 15 ماه بررسی شدند. بیماران با علایم و نشانه های دال بر آسیب داخلی جدی فورا عمل شدند. در سایر موارد، اندوسکوپی دستگاه گوارش فوقانی بکار رفت و بر اساس زمان آن، بیماران به دو گروه تقسیم شدند: اندوسکوپی زودرس (زیر 24 ساعت) و اندوسکوپی دیررس (بالای 24 ساعت). وضعیت بیمارستانی در دو گروه مقایسه گردید.100 بیمار، 62% مونث و 38% مذکر با متوسط سنی 25/951/25 سال وارد مطالعه شدند. بلع مواد سوزاننده در 53% موارد به قصد خودکشی و در 47% اتفاقی بود. مواد قلیایی در اغلب موارد (83%) و مواد اسیدی در 17% وارد بکار رفته بود. 8% موارد بر اساس علایم و نشانه ها فورا عمل شدند. 37% موارد تحت اندوسکوپی فوری و 55% موارد تحت اندوسکوپی تاخیری قرار گرفتند. در دو گروه اخیر نتیجه اندوسکوپی در 11% موارد طبیعی، در 40% موارد آسیب درجه I، در 28% موارد آسیب درجه II، در 7% موارد آسیب درجه III و در 6% موارد آسیب درجه IV بود. در دو گروه از نظر نیاز به عمل جراحی، فاصله پذیرش تا جراحی، نیاز به بستری در ICU و مدت آن و موربیدیتی و مورتالیتی طی بستری تفاوت معنی دار آماری وجود نداشت. با این وجود، فاصله تا جراحی و پذیرش در ICU در گروه اندوسکوپی فوری و موربیدیتی بیمارستانی در گروه اندوسکوپی تاخیری ظاهرا از وضعیت بهتری برخوردار بود
