بررسی آلودگی باکتریایی کاتترهای درون عروقی در بیماران بستری در مرکز آموزشی درمانی شهید مدنی

Abstract

The goal of this study was to evaluate the incidence of bacterial colonization of intravascular catheters and catheter related bacteremia in madanis educating and researching center. Methods & Materials In a prospective cross sectional study , 150 admited patients that had intravascular catheter more than 48 hours, were studied during and annual period, in heart surgery section and intensive care unit of madani hospital. The tip of catheters were cultured by using standard method, to study the isolated colonies. While bloodstream sample for blood culture was provided for patients, at the same time, other parts of body culture was used in some of the other patients. Bacterial colonization of the catheters, isolated microorganisms from catheters, incidence of catheter related bacteremia and risk factors of septicemia were studied . Results Form 150 studied patients , 102 patients(68%) were male and 48 patients (32%) were female, and the patients mean age was 44/68 23/51 years. From 150 cultured catheters , 84 catheters were CVP , 64 catheters were arterial and 2 of them were suan ganzes . 13 of the culture of catheters had positive results. Among these 13 catheters , 3 of them were arterial and 10 were CVP. The rate of isolated bacteries from these catheters were orderly: Escherichia coli( 23/1%)3, Pseudomonas aeruginosa(23/1%)3 , Staphylococcus aureus (7/7%) 1, Coagulase negative staphylococcus (7/7%)1 , Proteus vulgaris (7/7%) 1 , Stenotrophomonas maltophilia (7/7%) 1, Candida.spp (7/7%) 1, Nonfermentative gram negative bacilli (7/7%) 1 and Acinetobacter.spp (7/7%)1. , هدف از این مطالعه بررسی میزان کلونیزاسیون باکتریائی کاتترهای داخل رگی و شیوع باکتریمی ناشی از آن در مرکز آموزشی و تحقیقاتی شهید مدنی بود. مواد و روشها در یک مطالعه آینده نگر مقطعی 150 بیمار بستری در بخش های جراحی قلب و مراقبت های ویژه بیمارستان شهید مدنی که بیش از 48 ساعت کاتتر درون عروقی (وریدی و شریانی) داشتند، در مدت 1 سال بررسی شدند. از نوک کاتترها در محیط کشت مناسب طبق روش استاندارد کشت بعمل آمد تاکلونی های ایزوله شده احتمالی از آنها مورد بررسی قرار گیرد. هم چنین از بیماران نمونه خون محیطی همزمان جهت کشت خون تهیه شد و در بعضی از بیماران کشت از نقاط دیگر بدن نیز بطور همزمان انجام شد. میزان کلونیزاسیون باکتریائی، میکروارگانیسمهای جدا شده از کاتترها، شیوع باکتریمی ناشی از کاتتر و تعیین عوامل خطر سپتی سمی ها بررسی شدند. نتایج از 150 بیمار مورد مطالعه 102 نفر (68%) مذکر و 48 نفر (32%) مونث بودند و میانگین سنی جمعیت مورد مطالعه 51/23 68/44 بود. از 150 کاتتر کشت داده شده 84 عدد CVP، 64 عدد کاتتر شریانی و 2 عدد سوان گانز بودند که 13 مورد (7/8%) از کشت کاتترها مثبت بودند. از بین این 13 مورد کشت مثبت، 3 مورد از کاتترشریانی و 10 مورد از CVP جدا شدند باکتریهای ایزوله شده از کشت کاتترها به ترتیب شیوع عبارت بودند از:Escherichia coli (%1/23) 3 مورد، پسودومونا آئروژینوزا (%1/23) 3 مورد، استافیلوکوک اورئوس(%7/7) 1 مورد، استافیلوکوک کوآگولاز منفی (%7/7) 1 مورد، پروتئوس (%7/7) 1 مورد، Stenotrophomonas maltophilia (%7/7) 1مورد، Candida.spp (%7/7) 1مورد، Nonfermentative gram negative bacilli (%7/7) 1 مورد و Acinetobacter.spp (%7/7) 1 مورد.
