بررسی سطح فریتین مایع مغزی نخاعی برای افتراق مننژیت باکتریال و آسپتیک در بالغین

Abstract

this study diagnostic value of CSF ferritin for differentiation of bacterial meningitis was evaluated. During cross sectional study from 2008/3/5 for 2 years CSF was collected from 61 adult suspicious to meningitis .these patients admitted in Emam Reza and Sina hospital in Tabriz .CSF was analyzed for ferritin ,glucose, protein, cell count, culture and gram stain.CSF ferritin was measured quantitatively by immunoenzymometric assay through Monobind kit. clinical data was collected for any patient. collected data analyzed by SPSS software. Patients classified in three groups: bacterial meningitis, aseptic meningitis and non meningitis group. parameters analyzed and compared in three group by analysis of variance(ANOVA) and chi-square and Tooki test. cut off value for CSF ferritin with appropriate sensitivity and specificity extracted from ROC curve. From 61 specimen,19 case as bacterial meningitis ,19 case as aseptic meningitis and 23 case as non meningitis was classified .CSF ferritin in bacterial meningitis was 231,6361,27 ng/dl that was significantly higher than aseptic meningitis group.cue off value for ferritin was estimated 155 ng/dl with sensitivity 94% and specificity 21,7%.in this study symptoms such as level of consciousness and fever and seizure in differentiation of meningitis with different causes has not any value. but symptoms such as headache ,nausea and vomiting and neck rigidity was higher in bacterial and aseptic group than non meningitis group., است در این مطالعه ارزش تشخیصی فریتین مایع مغزی نخاعی برای افتراق مننژیت باکتریال بررسی شده است.طی یک مطالعه مقطعی به روش تمام شماری از تاریخ 15/12/1386 به مدت 2 سال مایع مغزی نخاعی از 61 فرد بالغ مشکوک به مننژیت پذیرفته شده در بخش های عفونی بیمارستان سینا و امام رضا تبریز از نظر فریتین، قند، پروتئین، آنالیز سلولی و کشت و رنگ آمیزی گرم بررسی شد . فریتین به روش کمی با استفاده از کیت های منو باند به روش سنجش ایمونو انزیمو متریک اندازه گیری شد.. داده های به دست آمده از مطالعه با استفاده از نرم افزار آماری SPSS مورد بررسی قرار گرفت. بیماران در سه گروه مننژیت باکتریال، آسپتیک و بدون مننژیت تقسیم بندی شدند. پارامتر های اندازه گیری شده در هر سه گروه با هم با استفاده از آزمون تحلیل واریانس یک طرفه و آزمون تعقیبی توکی و آزمون مجذور کای مقایسه شد. نقطه برش برای فریتین با حساسیت و ویژگی مطلوب از منحنی Rock استخراج شد.از 61 نمونه بررسی شده، 19 مورد مننژیت باکتریال،19 مورد مننژیت آسپتیک و 23 مورد بدون مننژیت تشخیص داده شد. میزان فریتین مایع مغزی نخاعی در گروه مننژیت باکتریال 27/6163/231 نانو گرم بر دسی لیتر بود که به طور معنی داری بیشتر از گروه مننژیت آسپتیک بود. نقطه برش برای فریتین 155 نانوگرم بر دسی لیتر با حساسیت 94% و ویژگی 7/21 به دست آمد. در این مطالعه علائمی مثل سطح هوشیاری و تب و تشنج ارزشی در افتراق بین مننژیت با علل غیر مننژیت نداشت. ولی علائمی مثل سردرد، تهوع و استفراغ و سفتی گردن در گروه مننژیت باکتریال و آسپتیک بیشتر از گروه بدون مننژیت بود.
