بررسی ارزش سونوگرافی در پیشگویی موفقیت در القای زایمان و مقایسه آن با نمره بی شاپ

Abstract

our target from this study is assessment the value of ultrasound in prediction of successful induction of labor and compare its results with Bishop score. In prospective descriptive study 200 patients from tabriz alzahra hospital during 1386-1387 selected. All of patients were singleton and had indication of pregnancy termination .their gestational age were 37-42 week and induction were done. Before induction of labor transvaginal sonography to measure of cervical length and posterior cervical angle and taransabdominal sonography to determine the position of fetal occiput was performed. Value of position of fetal occiput, cervix length and posterior cervical angle ,parity,maternal age and BMI of the induction-to-delivery interval,likelihood of vaginal delivery and likelihood of cesarean section separately assessed and statistical analysis was performed In occiput anterior and transverse position, significant prediction was provided by cervical length and partiy ,and in occiput posterior significant prediction was provided by posterior cervical angle ,parity,maternal age and BMI.likelihood of cesarean section for occiput posterior, anterior and transverse position predict significantly with cervical length,parity , maternal age and BMI.posterior occiput position assossiated with significantly reduced likelihood vaginal delivery within 24 h and a significantly increased likelihood of cesarean section.in prediction of vaginal delivery for specifity of 68% the respective sensivitities for ultrasound finding and the for bishop score it was 64% . the respective sensivitities for cesarean section were 73% and 53%, هدف از این مطالعه بررسی ارزش سونوگرافی در پیش بینی موفقیت در القای زایمان و مقایسه آن با نمره بی شاپ می باشد .در این مطالعه تحلیل آینده نگر 200 بیمار مراجعه کننده به بیمارستان الزهرای (س) تبریز در فاصله سال های 1387- 1386 جهت زایمان انتخاب شدند.همگی بیماران حاملگی تک قلو بوده و اندیکاسیون ختم حاملگی را داشتند . سن حاملگی انها 42 37 هفته بوده و القای زایمان انجام شد . قبل از شروع القای زایمان ، سونوگرافی ترانس واژینال برای اندازه گیری طول سرویکس و زاویه خلفی سرویکس و سونوگرافی ترانس آبدومینال جهت بررسی موقعیت سر جنین صورت گرفت. ارزش موقعیت سر جنین ، زاویه خلفی سرویکس ، طول سرویکس ، پاریته مادر ، سن مادر و شاخص توده بدنی مادر بر روی فاصله القا تا زایمان ، احتمال زایمان واژینال و احتمال سزارین بصورت جداگانه بررسی و نتایج تحت آنالیز آماری قرار گرفت.در موقعیت سر OAو OT ، طول سرویکس قبل از القای زایمان و پاریته پیش بینی مستقل معنی داری از احتمال زایمان واژینال ارایه می دهد و در موقعیت سر OP ، طول سرویکس، پاریته ، زاویه خلفی سرویکس و شاخص توده بدنی پیش بینی مستقل معنی داری را ارایه می نماید . احتمال سزارین به طور معنی داری برای موقعیت سر OT ، OPو OA با ارزیابی طول سرویکس قبل از القا، پاریته، سن مادر و شاخص توده بدنی پیش بینی می شود. موقعیت سرOP همراهی معنی داری با کاهش احتمال زایمان واژینال و افزایش احتمال سزارین دارد. در پیش بینی زایمان واژینال با ویژگی 68 درصد ، حساسیت یافته های سونوگرافی 78 درصد و نمره بای شاپ 64 درصد بود ه و حساسیت سزارین بترتیب ، 73 و 53درصد بود.
