تاثیر شستشوی پریتونئال با بوپیواکائین در کاهش درد شانه بعد از عمل جراحی کوله سیستکتومی لاپاروسکوپیک

Abstract

This study aims at evaluating the effect of peritoneal bupivacaine irrigation on postoperative pain after laparoscopic cholecystectomy In a double-blind, randomized clinical trial, 200 patients with acute or chronic cholecystitis candidates of laparoscopic cholecystectomy were evaluated in Tabriz Sina Hospital during a 15-month period. These patients randomized in two age and sex-matched 100-person groups. Peritoneal bupivacaine irrigation was carried out in the case group with 500 ml of normal saline containing 10 ml of 0.5% bupivacaine in lever surface and sub-hepatic region at the end of operation. In the control group similar approach was conducted by pure normal saline. Visual analogue pain scale (VAPS) was employed on hours 4, 12, 24 and 36 after operation for determining the intensity of pain in the two groups. There were 26 and 74 cases with acute and chronic cholecystitis in the case group and 35 and 65 cases in the control group, respectively (p=0.167). Seventy four patients complained of post-operative shoulder-tip pain, 34 and 40 cases in the case and the control groups, respectively (p=0.380). Postoperative frequency of visceral and incisional-related pain was, however significantly lower in the case group (59% vs. 75%; p=0.016 and 70% vs. 80%; p=0.001; respectively). The mean VAPS was significantly lower in the case group on all occasions measured after operation. Cumulative dose of analgesics was significantly lower in the same group after operation (1.390.49 mg vs. 2.090.47 mg; p<0.001). The mean duration of hospital stay was not significantly different in the two groups. There was no significant complication related to bupivacaine instillation in the case group., هدف از این مطالعه بررسی تاثیر شستشوی پریتونئال با بوپیواکائین در کاهش درد پس از کوله سیستکتومی لاپاراسکوپیک می باشد. در یک کارآزمایی بالینی دوسوکور، 200 بیمار با کوله سیستیت حاد یا مزمن که کاندید کوله سیستکتومی لاپاروسکوپیک بودند، بمدت 15 ماه در بیمارستان سینا تبریز بررسی شدند. این بیماران بطور تصادفی به دو گروه 100 نفری همسان از نظر سن و جنس تقسیم گردیدند. شستشوی پریتونئال با بوپیواکائین در گروه مورد با 500 میلی لیتر نرمال سالین حاوی 10 میلی لیتر بوپیواکائین 5/0% در سطح کبدی و ساب هپاتیک در انتهای جراحی انجام گرفت. در گروه شاهد روش مشابه با نرمال سالین خالص بعمل آمد. مقیاس آنالوگ چشمی درد (VASP) در ساعت های 4، 12، 24 و 36 پس از عمل جهت تعیین شدت درد در دو گروه بکار رفت.بترتیب 26 و 74 مورد با با کوله سیستیت حاد و مزمن در گروه مورد و 35 و 65 مورد در گروه شاهد وجود داشت (167/0= p). 74 بیمار در مجموع و 34 و 40 مورد بترتیب در گروه های مورد و شاهد از درد نوک شانه پس از عمل شاکی بودند (380/0=p). با این وجود، درد احشایی و محل زخم در گروه مورد کمتر بود (بترتیب 59% دربرابر 75%؛ 016/0=p، 70% دربرابر 80%؛ 001/0=p). متوسط VAPS در گروه مورد در تمامی زمان های اندازه گیری شده پس از عمل کمتر بود. دوز تجمعی ضددرد نیز در این گروه پس از عمل کمتر بود (49/039/1 در برابر 47/009/2 گرم‌؛ 001/0>p). مدت بستری بیمارستانی در دو گروه تفاوت معنی داری نداشت. عوارض عمده ای ناشی از شستشو با بوپیواکائین در گروه مورد مشاهده نگردید.
