بررسی ارتباط میان سطح سرمی پروتئین واکنشگر سی با حساسیت بالا (h.s.CPR) و پیش آگهی بیماران سکته مغزی ایسکمیک حاد

Abstract

Background and Purpose: Stroke is a syndrom that is ditincted with acute neurological defect at least for 24 hours. Ischemic stroke is the the most common type of stroke. C-Reactive protein (CRP) is acute phase protein that is released from hepatocytes at inflammation status. This study purpose is assessment of serum "High sensitivity C-Reactive Protein" level and prognosis of acute ischemic stroke. Methods: At this prospective descriptive analyzing study that is done at Imam Reza Hospital , we assessed patients with first ever ischemic stroke that have more than 40 year old. Two blood samples at 2nd and 5th day were taken and functional status were assessed with Modified Rankin Score (MRS) at 2nd ,5th day and 3th month after incidence of signs. Results: We assessed 102 patient (50 men,52 women) with mean 71.75 11.44 year old. "High sensitivity C-Reactive Protein"(hsCRP) mean at 2nd and 5th day were 11.61 9.45 and 14.68 11.86 respectively. Modified Rankin Score (MRS)mean at 2nd, 5th day and 3th month were 3.83 0/71, 3.92 1.05 and 4.171.97 respectively. Also hsCRP mean at diabet free members is higher than diabetic members at second day(P = 0.022). Three months prognosis was related with 2nd (P =0.000 , r = 0.43) and 5th (P =0.000, r = 0.74) day MRS and age of patients (P =0.000 , r = 0.409); but no association was seen between MRS at 3th month with 2nd(P =0.07,r =0.48) and 5th (P = 0.67,r = 0.041) day hsCRP serum concentration. There was no association between hsCRP and 3months prognosis after adjusting risk factors such as hypertention, hyperlipidemia and diabet mellitus ., هدف از این مطالعه بررسی ارتباط سطح سرمی این پروتئین با پیش آگهی بیماران سکته مغزی ایسکمیک حاد می باشد.مواد وروش : در یک مطالعه توصیفی تحلیلی آینده نگر که در بیمارستان امام رضا (ع) تبریز انجام گرفت، بیمارانی با اولین سکته مغزی ایسکمیک تایید شده که بالای 40 سال سن داشتند را مورد بررسی قرار دادیم. از بیماران 2 نمونه خون در روز های دوم و پنجم بعد از بروز علایم اخذ شد و توانایی عملکردی نیز توسط سیستم نمره دهی (MRS) Modified Rankin Scoreدر روز های دوم و پنجم وماه سوم بعد از بروز علایم ارزیابی شد.نتایج: تعداد 102 بیمار(50 مرد و52 زن) با متوسط سنی 44/11 75/71 سال مورد بررسی قرار گرفتند. میانگین "پروتئین واکنشگر سی با حساسیت بالا" (hsCRP) در روز دوم 45/9 61/11 در روز پنجم 86/11 68/14 بود. میانگین نمره ام آراس روز دوم 71/0 83/3 ، روز پنجم 05/1 92/3 و ماه سوم 97/1 17/4 بود. در بررسی بیماران میانگین "پروتئین واکنشگر سی با حساسیت بالا" در روز دوم در افراد غیر دیابتی بالاتر از دیابتی ها بود(022/0=P). ارتباط معنی داری بین ام آراس ماه سوم با ام آراس روز دوم (000/0=P ،43/0 =r )، ام آراس روز پنجم (000/0=P ،74/0 =r ) و سن(000/0=P ،409/0=r ) وجود دارد. ولی چنین ارتباطی در مورد ام آراس ماه سوم با غلظت "پروتئین واکنشگر سی با حساسیت بالا " در روز های دوم و پنجم دیده نمی شود؛ به ترتیب با (48/0=P ،07/0 =r )و(67/0=P ،041/0 =r ). بعد از تعدیل سایر عوامل خطر از جمله پرفشاری خون، دیابت شیرین و چربی خون بالا ارتباط معنی دار آماری بین ام آراس سه ماهه و غلظت "پروتئین واکنشگر سی با حساسیت بالا" در روز های اول دیده نشد.
