قرارگیری نامناسب پیچ پدیکول در ناحیه توراسیک تحتانی و لومبر و ارتباط آن با علائم

Abstract

The purposes of this study were to determine the incidence of screw misplacement, complications, the accuracy and usefulness of CT scan in evaluation of pedicle screw placement and to define the relation between the symptoms and the CT scan images. In a cross section and descriptive analytical study that performed in neurosurgery ward of Shohada hospital of Tabriz on patients with fracture and lystesis of thoracic and lumbar spine, incidence of pedicle screw misplacement and relation it with patients symptoms evaluated. Postoperative CT was performed in all 53 cases to evaluate implant position within first month after surgery. These images were then inspected for evidence of pedicle violation. From 53 studied patients, 23 patients were male and 30 patients were female. Mean age of studied patients was 39.09 years in range of 20-68 year. Surgical indication in studied patients included degenerative instability in 4 patients, Spondylolystesis in 28 patients, trauma in 18 patients and tumor in 4 patients. In assessing 247 pedicle screws inserted in 53 patients, lateral screw misplacement was observed in 59 screws(67.82%) and medial pedicle wall violation in 28 screws(32.18%). Of the 87 misplaced screws, 41 cases were classified as minor (cortical perforation <= 2mm), 41 cases as moderate (2.1-4 mm), and 5 cases as severe penetration (>4mm). Root injury with radicular pain and neurological deficits was observed in 8 patients with malpositioned screws (15.09% of all patients)., هدف از این این مطالعه تعیین انسیدانس قرار گیری پیچ، عوارض، و صحت و درستی و مفی بودن CT-Scan در بررسی جایگذاری پیچ پدیکول در معین کردن ارتباط بین علائم و تصاویر CT-Scan است.در یک مطالعه مقطعی توصیفی تحلیلی که در بخش نوروسرجری بیمارستان شهدای تبریز بر وری بیماران با شکستگی و لیستزیس مهره های توراسیک و لومبر انجام گرفت، بروز قرار گرفتن پیچ در محل نامناسب و ارتباط آن را با علائم بیماران مورد بررسی قرار دادیم. CT بعد از عمل در تمامی 53 مورد دبرای بررسی محل ایمپلنت ها در ماه اول بعد از جراحی انجام گرفت. تصاویر برای پیدا کردن شواهد تجاوز به پدیکول مورد بررسی قرار دادیم.از 53 بیمار مورد مطالعه 23 بیمار مرد و 30 بیمار زن بودند. میانگین سنی بیماران مورد مطالعه 09/39 سال در محدوده 68-20 سال بود. اندیسکاسیون های جراحی در بیماران مورد مطالعه شامل ناپایداری دژنرانتیو در 4 بیمار، اسپوندیلولیستزیس در 28 بیمار، تروما در 18 بیمار و تومور در 3 بیمار بود. در بررسی 247 پیچ پدیکول جایگذاری شده در 53 بیمار، جایگذاری نامناسب لترال پیچ پدیکول در 59 پیچ(82/67%) و تجاوز به دیواره میانی پدیکول در 28 پیچ(18/32%) دید ه شد. در 87 پیچ با چایگذاری نامناسب، 41 مورد در طبقه بندی خفیف(پروراسیون کورتیکال زیر 2 میلی متر)، 41 مورد متوسط(4-1/2 میلی متر) و 5 مورد بعنوان پرفوراسیون شدید(بیشتر از 4 میلی متر) قرار داشتند. آسیب ریشه عصبی همراه با درد رادیکولر و نقص عصبی در 8 بیمار با جایگذاری نا مناسب پیچ (09/15% کل بیماران) دیده شد.
