Effect of premedication with pharmapin and Dexamethasone on success rate of inferior alveolar nerve block for teeth with irreversible pulpitis

Abstract
Anesthesia in posterior teeth with Irreversible pulpitis harder than the other normal teeth. The aim of this study was to compare two kinds of anti-inflammatory medicines (dexamethasone and pharmapin) with a placebo according to their effects on the success rates of inferior alveolar nerve block (IANB) for endodontic treatment of mandibular molars with irreversible pulpitis. Methods: A total of 210 patients were divided into three groups of 70 patients each and were given a capsule of the same color and size (either a placebo of lactose powder, 400-mg pharmapin or 0.5-mg dexamethasone). After I hour of oral administration of the capsules, all the patients received standard IANB . In patients with successful IANB , the teeth were examined with cold pulp test; they were asked to assess their pain using the visual analog scale (VAS). Then, endodontic access cavity preparation was initiated . In case of pain during the treatment, the patients were asked to rate the pain on VAS . Success was defined as no or mold pain during treatment. Chi-squared test and ANOVA were used to compare qualitative and quantitative data between the groups. Results: No significant differences were found in sex and pretreatment pain between the patients in the three groups (p<0.981). The effect of Success of inferior alveolar nerve in teeth with irreversible pulpitis with 3 type of pro drug showed that the dexamethasone associated with a 51.4 % (less than 2.5). The Pharmapin was associated with 55.7% and Placebo was associated with 41.4 % . Conclusions: No significant differences were found in sex and pretreatment pain between the patients in the three groups (p<0.981). The effect of Success of inferior alveolar nerve in teeth with irreversible pulpitis with 3 type of pro drug showed that the dexamethasone associated with a 51.4 % (less that 2.5). The Pharmapin was associated with 55.7% and Placebo was associated with 41.4 % . The Chi-square test show that there is a signification deferens in the frequency of 3 type of drug(p=0.04). That rate of success of placebo group less than dexamethasone (p=0.041) and less than pharmapin (p=0.038) group but there are not significant difference between dexamethasone and pharmapin group (p=0.258).
