Sustained release of micro spherical Minocycline for management of peri-implant disease and its Cytokine profile

Despite the increasing prevalence of peri-implant disease; there is not much evidence to support available treatment options. The aim of current study was to assess the efficacy of local microspherical Minocycline on clinical parameters of inflammation and the quantity of cytokines Interleukin-6, Interleukin-17 in peri-implant crevicular fluid. Methods: Total of 46 patients with peri-implant mucositis or mild peri-implantitis with PD4mm, bon loss2mm, randomly received one of the two treatment protocols: Mechanical debridement alone (MD) as control, using ultrasonic scalar with metal tip and air powder polishing device, or MD followed by intra-sulcular application of micro spherical minocycline (MSD) as test group. Clinical measurements including Full Mouth Plaque Score (FMPS), Local Plaque Score (LPS), Probing Depth (PD), Bleeding On Probing (BOP), Pain On Probing (POP) and Suppuration (SUP) were carried out at baseline, 2 weeks and 3 month post-interventional. The Peri-implant Crevicular Fluid (PICF) was analyzed by enzyme-linked immune sorbent assay to quantify the Interleukin-6(IL-6) and Interleukin-17(IL17). Results: Twenty-two patients in MSD group, 21 in MD group completed the study. At 3 month follow-up, PD and BOP values in test group were reduced significantly (P<0.001) while in control group there were no significant changes in those values. Patients with POP in test group, were completely POP free (P<0.001). Quantity of Interleukin-6, Interleukin-17 was reduced significantly in test group (P<0.001) Conclusion: The adjunctive application of local microspherical Minocycline improved the clinical parameters and also reduced the Interleukin-6, Interleukin-17 quantities in PICF, for up to 3 month
