Comparision efficacy of crestal anesthesia and inferior alveolar nerve block for exodontia of posterior mandibular teeth
Abstract
The prevention of pain during dental treatment is the ultimate goal of all people who are working in this profession. This problem is more common in the mandibular block anesthesia. Problems because of anatomic variations, deep penetration and invasive needles, paresthesia, muscle lockjaw, Paralyze, oral transmission of micro flora to anatomic areas, the incidence of delayed numbness, hematoma formation, a high incidence of aspiration and difficult homeostasis in patients with bleeding usually occur in mandibular nerve block anesthesia. Purpose: This study compared the effectiveness of two methods of inferior alveolar nerve block and crestal infiltration anesthesia for mandibular posterior teeth before being removed. Materials and Methods:The study was performed interventional. Statistical population of the study included patients who were referred for extraction of mandibular posterior teeth.Patients who met the inclusion criteria were selected based on their examination. The statistical calculation was performed for 35 dental practices and crestal infiltration anesthesia and alveolar nerve block anesthesia were studied. Half of the patients in this study got infiltration into crestal injection in the right lower jaw and alveolar nerve block for the lower left mandible and on the other half of the patients it was vice versa; as a moderating factor for pain does not interfere with study results. For 2 methods of the anesthesia the onset of anesthesia, duration of anesthesia, blood pressure before and after injection, before and after injection rate and the number of consumer anesthetic cartridge, the number of consumed gauze packs to control bleeding after extraction and Possible side effects (dry socket and infection) and age and sex were recorded And the results were recorded in the attached form.the Statistical analysis of the variables and the results were compared for each method of anesthesia. Results:some variables like time before onset of anesthesia, duration of anesthesia and anesthetic cartridges used by T-test results found significant differences between the two groups block and infiltration .block group average was larger than average of infiltration group. (p<0.05)consumed gauze packs in the two groups based on test results mark a significant difference in the number of blocks and infiltration.(p<0.05) infiltration gauze packs consumption was lower than the block groups The non-parametric test results and changes in pulse rate between the two groups block and infiltration showed no significant difference. (p>0.05) Bp change in the sign test results between the two groups showed no significant difference. (p>0.05) To compare the two groups' pain intensity Sign test showed that there are significant differences between the two groups.pain intensity was lower in infilteration Group.(p<0.05) Conclusion:based on the results this technique could be favorable in mandibular posterior teeth extraction and other dental treatments and would be used instead of inferior alveolar nerve block Or as a complementary method to be used in cases where the block fails.
