Comparing the effect of one stage full mouth disinfection versus quadrant-wise root debridement on clinical parameters and salivary biomarkers in moderate to severe chronic periodontitis patients

Abstract
Elimination of dental plaque and calculus is considered as the main treatment option for inflammatory periodontal diseases. The aim of the present study was to compare the efficacy of one stage full mouth disinfection versus the quadrant-wise scaling and root planing in terms of clinical parameters and inflammatory biomarkers screening treatment of moderate to serve chronic periodontitis patients. Method and population: Forty patients, with generalized moderate to severe chronic periodontitis, participated in the study and were randomly and equally allocated to test and control groups. The test group received one stage full mouth disinfection (FMD) by ultrasonic tooth debridement in two visits within 24 hours in combination with irrigation of all pockets rinsing with a 0/2% Chlorhexidine solution. quadrant wise scaling and root planing (Q-SRP) with 2 weeks intervals was performed for the control group. At baseline, 2 months and 4 months, after the initation of the treatments, clinical attachment level (CAL), Plaque index(PI), Bleeding on probing (BOP), clinical probing depth (CPD) and modified gingival index(MGI) and salivary IL-1 and MMP-8 were measured using ELISA. Data were analyzed using descriptive methods (Mean +- SD) and ANOVA and paired t-test for intra-group comparisons with SPSS 16 software. Chi square test was used to test any significant intra group differences. Results: Both procedures resulted in statistically significant improvement in all of the measured parameters between the baseline and the 2 and 4 months (p<0.05%). No meaningful improvement was seen between the 2 and 4 months (p<0.05%) in both of the FMD and Q-SRP groups. There was no significantly difference between the study groups except for the MGI (P<0.05). Conclusion: Within the limits of this study, although the one stage full mouth disinfection improved the clinical parameters and salivary inflammatory biomarkers meaningfully, but no statistically significant differences were found between FMD and Q-SRP.
