Predicting early mortality in multiple trauma patients is crucial for effective management and resource allocation. The Terminal-24 score has been proposed as a tool for this purpose, but its efficacy compared to established trauma scoring systems needs further investigation. This study aims to evaluate the predictive value of the Terminal-24 score for early mortality within 24 hours among multiple trauma patients and to compare its performance with other trauma scoring systems. Methods and Materials: This study involved 963 multiple trauma patients, with a mean age of 37.75 years. Patients were categorized based on their survival status and time of death. Various trauma scoring systems, including Terminal-24, RTS, GAP, Revised GAP, and NTS, were compared using ROC curve analysis to determine their predictive accuracy for early and overall mortality. Sensitivity, specificity, and AUC values were calculated for each scoring system. Results: The Terminal-24 score demonstrated superior predictive performance for early mortality, with an AUC of 0.970, sensitivity of 96.9%, and specificity of 96.1%. In contrast, traditional scores like RTS, GAP, Revised GAP, and NTS showed lower AUC values and were less effective in predicting early mortality. Terminal-24 outperformed these systems in predicting both early and overall mortality, particularly in identifying patients at high risk of death within 8 hours of hospital admission.en_US
