Type 2 diabetes is a combination of metabolic syndrome (vasculitis, endothelial dysfunction, hypertension, dyslipidemia, and visceral obesity) and its pathophysiology is the result of a complex conflict between insulin resistance, defects in beta cells and adipokines . Insulin resistance exists in 90% of people with type 2 diabetes and research shows that 80% of these people are obese . Aim: The aim of studying the effect of hydroalcoholic extract of Fumaria parviflora on lipid profile, blood sugar indices, inflammatory factors, oxidative stress indices, in obese type 2 diabetic patients under low calorie diet. method: The current study was conducted in the form of a double-blind randomized controlled clinical trial on 106 women aged 20 to 50 (non-menopausal) and men aged 20 to 50 with a BMI between 25 and 40 kg/m2 with type 2 diabetes. The length of supplemental assistance was two months. Among 106 patients, 36 patients were initially excluded from the study. Finally, 70 people entered the study and were divided into two groups of 35 people. One group received only placebo (capsules containing placebo) corn starch, and the other group received 500 ml of Fumaria parviflora extract daily for two months. Three people from the group receiving Shahtare extract did not return at the end of the follow-up, and one person from the placebo group did not return. Finally, the data analysis was done between 32 people from the Shahtare extract group and 31 people from the placebo group. lipid profile, blood sugar indices, inflammatory factors, oxidative stress indices compare between two groups. findings: There was a significant difference in glycemic indices including FBS (p=0.008), HOMA-IR (p=0.003), QUICKI, (p=0.014) and insulin level (p=0.0011) as well as lipid profile including HDL-C. The serum levels of ALT, AST, ALP, BUN, urea, creatinine, as well as the ratio of HA1C and AST/ALT in the Fumaria parviflora group did not change significantly compared to the placebo group at the end of the trial. Anthropometric indices including body weight, BMI, WC and HC decreased significantly in both groups. Fumaria parviflora extract had a significant relationship with the reduction of biomarkers related to oxidative stress in patients with diabetes, including superoxidative, intracellular adhesion molecule, MDA, GSH-Px and TAC.en_US
