Abstract Introduction Family caregivers play an important role in taking care of their family. Caregivers face problems as a result of caregiving. These problems include a range of physical and mental problems. Stroke survivors live with severe disability. Caring for them has negative health consequences for the caregivers. Efficient and valid assessment tools enable early diagnosis for corrective action. Modified Caregiving Strain Index (MCSI) is a tool that measures caregiving Strain. This research was conducted for the first time with the purpose of psychometric questionnaire (MCSI). Materials: After cultural matching, questioning was done among 130 family caregivers who cared for the elderly with stroke. Content validity, face validity and construct validity were done. Principal component analysis with Promax rotation was used in exploratory factor analysis. The adequacy of the samples was used by the Kaiser-Meier-Alkin test (KMO) and the correlation of variables, Bartlett's sphericity. And dimensions were extracted with minimum eigenvalue of one and pebble diagram. In the confirmatory factor analysis stage, Lisrel software version 8 was used. The reliability of the instrument was evaluated through internal consistency by calculating Cronbach's alpha and test-retest Results Content validity was evaluated according to CVI-CVR index and face validity was confirmed. The instruments were grouped in one part (81.65% variance). According to Kaiser-Meier-Alkin (KMO) and Bartlett's sphericity tests, the samples were of sufficient volume (0.9)..The correlation matrix was also in the appropriate range (x2 = 640 and P < 0.000). The results showed that the questionnaire has excellent internal consistency (alpha coefficient = 0.94). The internal correlation coefficient for the entire questionnaire in test-retest was equal to ICC = 0.98, which has a high level of reliability. Conclusion This study showed that the Iranian version of MCSI has good psychometric properties and reliability. It is a valid tool for estimating care pressure.en_US
