Compilation and design of short and practical psychometric tools is one of the advantages and strengths of a good and standard test. The Delirium-O-Meter questionnaire is designed to measure the delirium status of patients. Based on this, the main purpose of the research was to check the reliability and validity of the Persian version of the Delirium-O-Meter questionnaire. Materials and methods: This study is an exploratory psychometric study that was conducted in 2021-2022 in the medical research training centers of Tabriz University of Medical Sciences. The statistical population of the study included all patients admitted to this medical research training centers with the diagnosis of delirium. 40 patients were selected in the phase of determining the reliability of the questionnaire and 110 patients were selected to determine the construct validity of the questionnaire. In addition, the face validity and content of the questionnaire were determined with the participation of 10 nurses and 10 psychiatric experts. The sampling method of patients was done in an accessible way and the sampling method of experts and specialists was done in a targeted manner. The Persian version of the Delirium-O-Meter questionnaire was used to collect data. Exploratory and confirmatory factor analysis test, Pearson correlation and Cronbach's alpha were used. Statistical calculations were performed using SPSS version 18 and Smart PLS software. Findings: The average impact coefficient of the items was 3.24, the average CVR was 0.78, and the average CVI was 0.87. Cronbach's alpha coefficient for the whole questionnaire was 0.77. The value of KMO index was equal to 0.88 and the significance level of Bartlett's sphericity test was equal to 0.001. In total, from the 11 questions in the questionnaire, 2 factors were extracted that explained 76.09% of the total variance. The Pearson correlation coefficient between the two stages was calculated for the overall score equal to 0.91.en_US
