Evaluation of anesthetic efficacy of incisive nerve block with articaine 4% and epinephrine 1.200000 compared to inferior alveolar nerve block with lidocaine 2% and epinephrine 1.80000 during mandibular second premolar extraction
Abstract
The aim of this study was to compare the effect of incisive block anesthesia with articaine 4% with epinephrine 1.200000 and lower alveolar block with lidocaine with epinephrine 1.80000 during the extraction of mandibular premolar teeth.
materials and methods
In this double-blind split-mouth clinical trial study with IRCT code 20230128057259N1, 42 patients who needed the extraction of second premolar teeth on both sides of the lower jaw and who met the criteria for entering the project were selected. After obtaining the consent form, the second premolar tooth was extracted on one side of the patient's jaw using the lidocaine lower alotol block method. After one week, the second premolar tooth on the opposite side was extracted using the articaine incisive block method. In order to check the effectiveness of two types of anesthesia methods, three factors: the time to reach anesthesia (lip anesthesia), the need for re-injection of anesthesia, measuring the amount of pain with the VAS scale immediately after the extraction of each tooth. It was done in both stages by the same person and under the same conditions. SPSS version 24 software and Mann-Whitney statistical test, Chi-square test and Fisher's exact test were used for data analysis.
Results
In first and second premolar teeth, anesthesia time in incisal block with articaine 4% was significantly less than lower alveolar block group with lidocaine (P<.005). The amount of pain in the incisive block group was significantly lower than the inferior alular block group. (P<.005). The need for secondary anesthesia was lower in the incisive block group than in the lower alular block group (P<.005).
Conclusion
The incisive block method with 4% articaine can be used in the extraction of mandibular premolar teeth instead of lower alveolar nerve block.
