Heart failure is a common epidemic disease which is increasing in prevalence by every decade after 50’s. It is important to be familiar with heart failure and it’s prognosis regarding to senile population. In recent decades, mortality causes shifted from infectious diseases to diseases related to life style such as cardiac disease, cancer and diabetes. Methods: This retrospective study included 767 heart failure patients above 18 years with ejection fraction less than 40 percent who were admitted to Tabriz Shahid Madani Hospital in1399-1400 .Data were analyzed by descriptive and analytic statistics.Exclusion criteria were dialysis and chronic kidney disease history. Independent variables were serum sodium, serum creatinine and sodium to creatinine ratio. Dependant variable were readmission and 1-year mortality. Finding: This study showed that the highest mortality rate and readmission was 62.5 percent in sodium range 120-125mg/dl. Highest mortality rate due to creatinine range was 35 percent in creatinine range more than 3mg/dl. Mortality rate due to sodium to creatinine ratio showed overall descending pattern with higher range.en_US
