Evaluation the predictive value of mean platelet volume (MPV) in the prognosis of ischemic stroke patients receiving intravenous tissue plasminogen activator (tPA)

Abstract
Cerebrovascular diseases in Iran are considered as the second cause of death and in terms of disease burden, it ranks sixth among diseases. A stroke is a neurological disorder caused by a cerebral blood flow disorder that lasts for more than 24 hours. Thrombolytic therapy (with recombinant tissue plasminogen activator [tPA]) is an effective treatment for acute ischemic stroke. In this study, we investigate the value of mean platelet volume (MPV) in the prognosis of ischemic stroke patients receiving tPA. Methods This descriptive non-interventional study was conducted retrospectively and it was based on the recorded data of patients with acute ischemic stroke who received tPA from three research educational centers of Imam Reza in Tabriz, Valiasr Training Center in Zanjan and Buffalo Hospital in the United States. The data was collected from the clinical files of the patients. The diagnosis of acute ischemic stroke was made based on the international neurology guidelines and on the basis of history, examination, brain CT scan (MRI if necessary) and ultrasound of the carotid and vertebral arteries. SPSS statistical software was used for statistical analysis of the study. Results In this study, the information of 2466 patients in three centers were used. The most common type of stroke in Zanjan Center was Large Vessel and Cardioembolic in Tabriz Center. MPV was not significantly related to the type of stroke in patients in Tabriz (p=0.122) and Zanjan centers (p=0.465) and in the total of patients in these 2 centers (p=0.705). There was no significant relationship between age and MPV rate in any of the centers. In the study of the relationship between sex and MPV, a significant relationship between these 2 variables was found in the American Buffalo Center (p=0.032), while this relationship was not significant in Tabriz and Zanjan. Examining the relationship between complete recovery of patients (based on mRS) and MPV was not associated with significant results. In Logistic Regression analysis, there was no significant relationship between MPV and complete recovery of patients.
