Title: Prognostic value of percent Gleason Grade 4 and 5 versus conventional Gleason score in prostate needle biopsy on predicting prostatectomy pathology and recurrence. Agajanzadeh.B.MD-Shokouhi Ghogani.B.MD Pathology departement, Imam Reza Hospital, Faculty of Medicin, Tabriz University of Medical Sciences Introduction: In the current prostate cancer grading method, the percentage of different grades is not determined, but only the presence of each grade is checked. According to new studies, the problem of prognostic differences in different percentages of each grade has been raised and it seems that the relationship between the percentage and prognosis of pathology after prostatectomy and disease recurrence is more and more effective than the conventional method of scoring Which requires further studies to investigate this issue. Material and method: Among the prostate needle biopsy specimens received in the pathology laboratory of Imam Reza (AS) Hospital in Tabriz during 2015, which had a diagnosis of prostate cancer, 50 cases were randomly selected. Re-microscopic examination of cancer samples was performed using two conventional scoring methods and a new method for determining the percentage. These patients were retrospectively studied over three years and the surgical pathology and recurrence of the disease were evaluated. Result: The mean age of patients was 67 years. The most common Gleason score was 3 + 4 = 7 (32%). The highest number of patients with preoperative PSA was 4.1-10(ng/ml).Adverse pathology was present in 38 patients (76%) after prostatectomy. In 62.5% of patients with recurrence of the disease, the percentage of 4/5 Gleason was between 70-100%. There was a statistically significant relationship between the percentage of 4/5 Gleason with recurrence of the disease and adverse pathology (P˂0.001).en_US
