Evaluation and comparison of the results of acute subdural hematoma evacuation by two methods of multidural stabs versus open dural flap

Abstract

Traumatic subdural hematoma is one of the lethal injuries to brain. Various surgical techniques are used to evacuate the acute subdural hematoma. The hematoma evacuation can either be done by opening of dura by multiple slits or by opening of dura in single large c shape and then doing the expansile duraplasty. The aim of this study was to compare the two methods of multidural stabs and Open-dural flap. Methods: This randomized clinical trial study was performed in the neurosurgery ward of Imam Reza Hospital in Tabriz from July 1998 to March 2010. During treatment, a questionnaire related to postoperative complications such as infection, cerebrospinal fluid leakage and length of hospital stay, intraoperative bleeding and herniation, GCS, GOS were recorded in both groups. The collected information was analyzed using SPSS software version 22. Results: Patients did not differ significantly in terms of demographic characteristics. The most common complication seen in Open-dural flap, cerebral hernia and postoperative bleeding in 40% of patients. And in the multidural stabs group, CSF leakage was present in 30% of patients. The Glasgow Outcome Scale was not statistically difference between the two groups and most patients in both groups had GOS4.
