Fat accumulation in the liver (fatty liver) as a factor leading to the disease has different consequences and causes. Since shear wave elastography is a new method in examining liver tissue, so the aim of this study was to compare the diagnostic value of ultrasound and MRI in patients with fatty liver. Materials and Methods: This cross-sectional study was performed on all patients with fatty liver who are referred for abdominal MRI for any reason. In patients undergoing MRI, the In and Out phases of fatty liver imaging were performed and cases of non-infection were also recorded and recorded. The subjects were then sonographed and graded, and their findings were recorded and stored. Ultrasound blind without knowing the result reported on MRI; It was written by a radiologist and the final results were compared. In the present study, according to the radiology reference after biopsy, which is the main gold standard and was not used due to the invasive method; MRI was used as the standard gold method for the diagnosis of fatty liver, cirrhosis and staging. The subjects then underwent elastography and ultrasound, and their staging was recorded and stored, and the final results of the three were compared. Also, the sensitivity and specificity of elastography and ultrasound in comparison with MRI in the diagnosis of fatty liver were calculated. Results: In this study, 49 patients with fatty liver were studied. The mean age of the patients was 57.97±15.56 years. According to MRI, 36.7% of patients were positive for fatty liver. The mean difference of In-Out phase in MRI modality in positive cases was 58.83±47.70. Gray Scale ultrasound with a cut-off point of 1.255 for B-Ratio had a sensitivity of 83.3% and a specificity of 71%. Also, elastography sonography with a cut-off point of 6.85 had a sensitivity of 61.1% and a specificity of 33.3%.en_US
