Introduction: Some complications including infection may occur following surgery of impacted teeth. The present study aimed to evaluate infection and affecting factors in hard tissue impacted mandibular third molars surgery in patients referred to the Oral and Maxillofacial Surgery Department and the specialized dentistry clinic of Tabriz University of Medical Sciences, Iran from September 2020 to May 2021. Methods: This descriptive-analytical cross-sectional study included 148 patients. The tool was a researcher-made checklist one week, two weeks, and one month after surgery. Data were entered into SPSS software version 17 and analyzed using descriptive statistics and the Mann-Whitney U test, Pearson’s chi-square test, and logistic regression model. A P-value less than 0.05 was considered a statistically significant level. Results: Out of 148 patients, 55 (37.2%) were male and 93 (62.8%) were female. EOI occurred in 5 (3.4%) operated patients. After four weeks of surgery, none of the patients showed signs and symptoms of periosteal infection. Statistical analysis showed that, among the predictor variables, the occlusion difficulty according to Pederson's classification (P=0.013) and surgeon's experience (P=0.028) were associated with postoperative infection (P<0.05). Conclusion: Our results indicated that among the predictor Variables, the occlusion difficulty according to Pederson's classification and the surgeon's experience were associated with the risk of postoperative infection in the mandible.en_US
