Chronic obstructive pulmonary disease (COPD) is a disease with limited airflow that is not completely reversible. Patients' quality of life is associated with acute exacerbation. Occasionally, cardiac troponin I, is also positive during acute exacerbation and may have a prognostic role in COPD in addition to heart disease. Methods: Ninety patients with definitive COPD were included in the study. Demographic data, physical examination results, ECG and CXR report, other paraclinical tests and serum CTNI were measured (during 24 and 48 hours after admission). Patients were divided into two groups according to CTNI values (positive cTNI: ≥ 0.3 ng/dl and negative cTNI: <0.3 ng/dl) and Then its relationship with hospitalization, mortality and need for mechanical ventilation and CPR were investigated. Results: Hospital mortality rate (8.88% vs. 4.4%) and mortality at 30 days (6.66% vs. 3.3%) were higher in patients with positive CTNI in the first 24 hours (P = 0.001). The duration of ICU admission (37±14.61 vs 9.83±4.93 days) and CPR (10% vs. 5.5%) were higher in patients with positive CTNI in the first 24 hours (P<0.05). The number of cases requiring intubation in patients with positive CTNI during 24 hours was significantly higher (100% vs. 12.3%, P = 0.001).en_US
