Introduction: Unfractionated heparin is stay widely utilized in treatment of ischemic heart disease. Since the failure to achieve sufficient unfractionated heparin levels impairs its antithrombic efficacy, hence it's high levels of unfractionated heparin can cause excessive bleeding. Therefore, laboratory monitoring is needed to achieve the anticoagulant effect of heparin. Aim: To assess the rate of control of PTT in therapeutic range and the risk factors that is associated to uncontrolling of PTT. Methods: One hundred and two patients were included in this prospective, case series study conducted for 6 month to evaluate the extent of aPTT in therapeutic range according to the standard protocol. The data included demographic details besides all information regarding treatment and results of heparin therapy from patient’s medical records. Results: Percentage of Mean PTT in therapeutic range was 37.3%. Mean PTT was 44.4±14.4 seconds. Average days of heparin administration were 7.5 days. Average day that heparin in therapeutic range was 3 days. Mean PTT was significantly associated with age, DH, PTT in range, Days in control in therapeutic range, Percentage of days that heparin in therapeutic range, PT, INR and reversed association with smoking, TG, Hgb. Conclusion: The results showed that no studied patients have not been reached to the target aPTT during hospitalization period. Quickly achieving and maintaining a therapeutic aPTT is crucial for attaining optimal treatment outcomes and prevention of potential complications of heparin therapy. The study highlights the importance of following the guidelines for appropriate use of unfractionated heparin.en_US
