Administrative Barriers to Promote Continuous Quality Nursing Care from the Perspective of Nurses in Health Centers of Tabriz University of Medical Sciences, 2002.
Abstract
Abstract: Introduction: CQI (continuous quality improvement) is an approach that occupied industry for recent decades which gradually distributed to health care services especially nursing care. In resent as, health care authorities have put so much effort in to improvement nursing services quality, but failed to get the goal because of obstacles. Such empowerment could have worked to decrease health costs, relief patients and revert safe people to society. The study is run by nurses working in health centers of Tabriz University Medical Sciences in an effort to discover obstacles.
Methodology: This study is a descriptive –analytic the run by nurses working in health centers of Tabriz University of Medical Sciences in an effort to discover obstacles. The samples consist of 238 nurses and sampling procedure were objective and data collection instruments were questionnaire.
Findings: Findings shows most of samples were female 62/2%, married 60/5%, employed 77/7%, B.s holder 94/1%, at age group 35-39 years 22/7%, service record 5-9 years 23/5%, without management experience 70/2% & attending CQI classes 35/3%. Also the most of subject believe the most obstacles at strategic programming, lack of clear QI objectives 76/58%, most obstacles at training of staff, improper evaluation of staff skill and knowledge 78/9%, most obstacles at senior managers commitment, lack of support senior managers for QI efforts 75/2%, most obstacles at customer, not using patients option 64/7%, most obstacles at team working, unclear job description of the staff members in providing services 64/7%, most obstacles at staff participation, lack of enough power & authority by managers 72/2% and most obstacles at communication, improper use of communication technologies at work process 69%. Most obstacles was strategic programming. Results shows significant relationship between variables of service record and staff of training (p=0/0009) and between variables of service record and senior managers commitment (p=0/21). No other significant relationship could be detected.
Discussion: Obstacles facing CQI exaction have unfortunately, has caused to lower efficacy and effectiveness of nursing services because of the complicated nature of service delivery in health centers. Therefore nursing authorities to understand these obstacles could way for more efficient CQI in nursing care, otherwise mentioned obstacles cause health danger to patients and increase health cost. Recognizing these obstacles and effective variables would reduce health costs.
