Introduction: Despite the efforts of many organizations and experts to reduce the excessive use of antibiotics in hospitals, unfortunately, this trend continues. The aim of this study was to evaluate the effectiveness of the monitoring strategy for the administration of broad-spectrum and costly red line antibiotics in Imam Reza Educational and Medical Center in Tabriz in 2009. Materials and Methods: In the present study, 9 broad-spectrum red line antibiotics including: vancomycin, teicoplanin, imipenem, meropenem, amphotericin B liposomal, voriconazole, caspofigin, calcitine, linezolid were evaluated. Case data of hospitalized patients who received the above antibiotics were extracted. Clinical records that were admitted to Imam Reza Educational and Medical Center in Tabriz during 2009 were extracted. Results: In this study, a total of 236 patients who received broad-spectrum antibiotics and red line in Imam Reza (AS) Hospital in Tabriz in 2009 were studied. Of these, 161 cases (63.2%) were men and 75 cases (36.8%) were women. The mean age of these patients was 23.1±46.6 years and they were in the age range of 1 to 94 years. The most common antibiotics used were vancomycin, carbapenem (imipenem-meropenem) and cleistine, which accounted for 83.05%, 39.4% and 7.20%, respectively. In 213 cases (90.3%) the antibiotic dose was correct and in 23 cases (9.7%) the antibiotic dose was received incorrectly. Finally, in 153 cases (64.8%) of patients, the method of prescribing antibiotics was in accordance with the guideline and the antibiotic composition was observed correctly, and in 72 cases (30.5%) was not in accordance with the guideline. Conclusion: It seems that the implementation of the red line antibiotic monitoring strategy has not been as successful in controlling the irrational administration of these drugs as expected, and needs to be reviewed.en_US
