dc.description.abstractBackground: Heart failure is one of the most important causes of mortality and morbidity in both developed and developing countries. Lots of studies have been conducted for finding superior objective and laboratory methods for the diagnosis of this disease. Recently the role of miRNAs has been highlighted and a variety of miRNAs have been suggested for diagnosis of this disease. Of the most important miRNAs are miRNA-423 and miRNA-1. Through this study other than evaluation of diagnostic role of these miRNAs, the prognostic role of these miRNAs was also evaluated. Methods & Materials: Through a case-control study, an overall 102 blood samples were obtained from patients admitted with a diagnosis of acute heart failure in Tabriz Shahid Madani hospital and healthy control group. Then blood plasma was extracted and evaluation for miRNA-1 and -423 was conducted by means of real-time PCR. Thereafter, patients were followed for one year and clinical data od their admission time and one year after discharge were collected and analysis of the data was done by SPSS version 22. Results: An overall 102 blood plasma samples were evaluated including samples from 44 patients with heart failure (43.5%) and 58 controls (57.5%). There was no significant difference in demographic characteristics between the two groups. Mean miRNA-1 in Heart Failure patients was 1.93 +/- 0.83 which was significantly higher than control group (p value<0.0001). In evaluation of miRNA-423, mean miRNA-423 was 1.681 +/- 0.546; which was significantly higher than the control group (p value<0.0001). In patients’ follow up, 35 patients were followed (9 patients (20%) were not available). Of 35 patients that were followed, 10 patients had died (28.6%). An evaluation of prognostic value of miRNA-1, analysis of data revealed that regarding ICD implementation, patients with higher miRNA-1 had higher rate of ICD implementation (p value=0.029). Also, miRNA-423 was significantly higher in patients who died during one year after discharge as compared to alive patients (p value= 0.048).en_US
