Objectives: to compare two non-invasive respiratory supports NCPAP vs. NIPPV in the preterm infants28-32week with respiratory distress syndrome. Methods: This randomized clinical trial study was performed in Tabriz Al-Zahra Hospital. Premature neonates with 28- 32 week gestation, suffering from RDS entered the study. including 66 newborns admitted to Al-Zahra NICU during 2015-2016. Most of these patients received surfactant as early rescue via INSURE method and then randomly divided into two NCPAP (31 neonates) and NIPPV (29 neonates) groups. Both early and late complications are compared including : hospital length of stay, chronic lung disease(CLD), GI perforation, apnea, intraventricular hemorrhage (IVH), mortality rate and intubation rate. Resulte: The rate of respiratory failure in the NIPPV (16.7%) vs NCPAP (6.5%) with (p=/255). The need for surfactant in the NIPPV (50%) and NCPAP (63.3%) with (P =.375) No significant difference was found for the need for surfactant. The most important complication was in both groups of apnea, with (33.3%) in the NIPPV group, and (19.4%) NCPAP, (P=/255). The most significant difference between the two methods of ventilation was the reduction in the hospital length of stayin the NIPPV compared with the NCPAP (26.2 + 26.4 + vs. 38.4 + 19.3) with (P = 0.009). For other complications in this study There was no significant difference between the two groups.en_US
