Introduction: The importance of preserving primary molars even if necrotic, especially in the second molars before the first permanent molars is obvious. Removing of microorganisms during pulpectomy treatment of primary molars is effective in treatment success. Due to the presence of formaldehyde and its risk of being carcinogenic in the formocresol substance, which is commonly used in pediatric pulpectomy, it was decided to use ozone gel, which has recently had successful medical and dental results in pulpectomy. Materials and Methods: 64 teeth were randomly divided into two groups of case (ozone gel) and control (formocresol). After access, canal cleaning, filing and washing, in the case group, the ozone-gelled cotton and in the control group, formocresol were inserted into the pulp chamber and dressed with zonalin and refilled with ZOE approximately one week after dressing with ZOE, and the crown was restored with amalgam or SSC. Clinical and radiographic success was assessed by two pediatricians after 6 months. Results: Comparison of clinical success rates in the two groups was similar in groups of formocresol (93.33%) and ozone gel (85.71%). Comparison of radiologic success rate was similar in the two groups of formocresol (90%) and ozone gel (85.71%). Also in the ozone gel group, clinical success (85.71%) and radiography (85.71%) were similar. In the formocresol group, clinical success (93.33%) and radiography (90%) were similar. Conclusion: The use of ozone gel in comparison with formocresol was not significantly different in clinical success and pulpectomy radiography of necrotic primary molars in 6 month follow up and could be a good alternative to formocresolen_US
