The aim of this study is to investigate the correlation of corneal thickness and corneal higher order aberrations. Methods & Materials: One hundred and sixty three eyes of keratoconus patients enrolled in this cross sectional study (53 eyes mild KCN, 59 eyes moderate KCN, 51 eyes severe KCN).corneal thickness and corneal higher order aberrations and correlation of them as main outcomes of study recorded. The comparisons were done with Pearson correlation test, chi square test, one way ANOVA, Kolmogorov-Smirnov Test and ROC.statistical significance was considered at p< 0.05. Results: Patients mean age was 26±6/93( 12,39 ) years. Mean UCVA and BCVA was 0/8± 0/4 and 0/34±0/34 (log MAR).mean refraction sphere and cylinder was -3/53± 3/35, -4/37± 2/49 diopter. Mean of corneal thickness (thinnest location) in mild, moderate and severe stages was significantly different (474/71± 49/1, 446/28±33/38, and 395/66±46/42). Mean LOA RMS (181/9± 7/99, 193/76± 25/11, 233/54± 28/12) , HOA RMS (3/59 ± 1/41, 7/35 ± 1/76 and 13/65 ± 4/48) was significantly different in stages.(p< 0.001).in each group stage of disease corneal thickness was significantly correlated with corneal lower and higher order aberrations.(p<0.05).Proposed cut off point were 6.07 for RMS HOA (77.6% & 75.4%) , -1.26 for Vertical Coma (75.5% & 68.4%) and 0.08 for Spherical Aberration (87.4% & 41.2%).en_US
