dc.description.abstractIntroduction: Vitamin D is a fat-soluble vitamin that plays a serious role in calcium homeostasis and bone metabolism also it has consequences on the immune and cardiovascular systems. Vitamin D plays a role in host defense and vitamin D deficiency is known to be associated with mortality in ICU patients. Vitamin D deficiency is frequently seen in public also in critically ill patients. APACHE IV score is the worst conditions that patients have at ICU admission in the first 24 hours. Patients with higher APACHE IV score have worse conditions. Also low serum vitamin D level is described to be associated with increased APACHE IV score. Aim: our goal was to determine the relationship between vitamin D level and APACHE IV score at ICU admission. Methods: A total of 300 patients arrived to the ICU between September 2016 and September 2017 were assessed in a prospective study. 84 patients were excluded because their serum vitamin D levels were not available. We collected vital parameters of ICU patients. Age, sex, APACHE IV score, serum vitamin D level, ventilation, outcomes were analyzed by using Pearson Correlation of SPSS software package (Ver. 22). Result: 43.88% of patients had sever vitamin D deficiency. Patients with low serum vitamin D level significantly had increased APACHE IV score. (p<0.05) Patients with low GCS seriously had vitamin D deficiency. Conclusion: Low serum vitamin D level was seriously correlated with increased APACHE IV score. Also vitamin D deficiency was significantly related to poor outcomes and decreased GCS. Future prospective studies are needed to confirm our findings and to determine whether the use of vitamin D can effect clinical management.en_US
