Left main coronary artery lesions are dangerous due to its anatomic pathology. Significant LMCA disease occurs in 5 to 7 percent of patients undergoing coronary angiography. In this study we are investigating the effect of the two main treatment methods Coronary angioplasty and CABG for the patients with LMCA. Method and Matherial: We investigated 240 patients whose LMCA problem diagnosed by coronary angiography. This group of individuals have been treated by two methods of Coronary angioplasty(PCI) and CABG with the population of 99 and 141 individuals respectively. the primary endpoints are defined as death, Myocardial Infarction, and secondary endpoints are defined as Target Lesion Revascularization(TLR) and Readmision . Additionally, the SYNTAX SCORE and Coronary Angiography Details have been reported. Findings: Mortality rate in PCI group are 6.06 percent, and in CABG 5.1 percent, with no significant difference. Also, there was not any significant difference between two groups for the Myocardial Infarction (p=0.985). Hospital Readmissin rate for of Coronary angioplasty and CABG arms reported to be 22.2 and 12 percent. This shows a no noticeable difference between two methods. (P=0.08). TLR was occurred in 7.6 and 6.3 percent of PCI and CABG groups respectively (p-value=0.784)Which didn’t show significant difference. Also, for the group of individuals who had CABG, the mean of Syntax-Score-I is significantly higher than the other group who had Coronary angioplasty. Mean values for first group is defined as 31.3±10.6 and for the second group is 22.6±7.7. P=0.009.en_US
