بررسی آینده نگر رژیم های پروفیلاکسی آنتی بیوتیکی در بیماران تحت اعمال جراحی قلبی در بیمارستان شهید مدنی تبریز در سال تحصیلی 95-1394

Abstract

مقدمه پروفیلاکسی آنتی بیوتیکی قبل اعمال جراحی قلبی، همچون سایر جراحی ها نقش مهمی در پیشگیری از عفونت دارد. به طوری که مصرف غیر منطقی آنتی بیوتیک ها باعث ظهور میکروارگانیسم های مقاوم نسبت به آنها و بروز عفونت های خطرناکی چون مدیاستینیت شده است که در نتیجه آن میزان مرگ و میر افزایش می یابد.هدف ارزیابی مصرف رژیم پروفیلاکسی آنتی بیوتیکی در بیماران جراحی شده در بیمارستان قلب و عروق شهید مدنی تبریز و انطباق آن با گایدلاین ASHP.روش کار این مطالعه طی یک دوره 8 ماهه و به صورت حال نگر در بیمارستان شهید مدنی تبریز بر روی بیمارانی که تحت عمل جراحی قلبی قرار گرفتند، انجام گرفت. اطلاعات دموگرافیک و سابقه بیماری های زمینه ای و رژیم آنتی بیوتیک پروفیلاکسی در پرسشنامه مربوطه ثبت شد و نحوه تجویز آنتی بیوتیک پروفیلاکسی با پروتکل های معتبر جهانی مانندASHP مقایسه شد.یافته ها 200 بیماری که تحت عمل جراحی باز قلبی قرار گرفتند، مورد مطالعه قرارگرفتند. 5/38 درصد بیماران را زنان و 5/61 درصد آن ها را مردان تشکیل می دادند. جراحی بای پس عروق کرونری معمول ترین جراحی انجام شده بود. 50 درصد از رژیم های پروفیلاکتیک آنتی بیوتیک شامل سفازولین بود. بیشترین خطا ها مربوط به دوز تجویزی قبل عمل، طول مدت پروفیلاکسی و تکرار دوز آنتی بیوتیک حین جراحی بودند که به ترتیب در 100، 98 و 71 درصد موارد با دستورالعمل مطابقت نداشتند. 26 درصد بیماران علایم عفونت داشتند. 8 بیمار با مورد مدیاستینیت بستری بودند.نتیجه گیری طی بررسی روند تجویز رژیم های آنتی بیوتیک پروفیلاکسی همخوانی ضعیفی با گایدلاین ASHP مشاهده شد. اضافه شدن متخصص داروسازی بالینيِ آگاه به دستورالعمل های رسمی و به روز به تیم جراحی جهت بهبود دارودرمانی، افزایش مصرف منطقی دارو و کاهش عفونت های بیمارستانی توصیه می گردد., Introduction Antibiotic prophylaxis before open heart surgeries plays a pivotal role in the prevention of infection. Inappropriate antibiotic prophylaxis may lead to life-threatening infection such as mediastinitis and development of resistance micro-organisms which affects treatment outcome and increases morbidity and mortality.Aim Evaluation of antibiotic prophylactic regimens in patients undergoing cardiothoracic surgery in Shahid Madani Hospital in Tabriz and comparing data with ASHP standard guidelines.Methods The present study is a prospective cross-sectional study which was conducted during an 8-months period on patients undergoing open heart surgery in ICU of Shahid Madani Heart Center affiliated to Tabriz University of Medical Sciences. All patients medical file were assessed regarding to prophylactic antibiotic regimen. Demographic information and history of background diseases and drugs as well as dosage protocol, type of the prophylactic antibiotic and antibiotic duration were assessed and compared to the ASHP standard protocol.Results Among 200 patients were assessed, 38.5% of patients were women and 61.5% were men. CABG was the most common type of surgery done in this center. Cefazoline was the routine antibiotic prophylactic regimen (50%), which was appropriate regimen according to ASHP guidelines. Errors in prophylaxis duration (98%), preoperative antibiotic dose (100%) and repeat dosing (71%) were common and very high. 58% of preoperative antibiotic administrations before incision were adequate. 26% patients had infection signs and symptoms. 8 mediastinitis cases were reported during 8 months.Conclusions In our study, prophylactic antibiotic regimen adherence with ASHP guidelines was poor. And the rate of life-threatening infections were high and alarming. This issue shows the necessity of a clinical pharmacists in hospitals who are aware of the new reliable guidelines.
