بررسی دیدگاه انترن ها نسبت به خطاهای پزشکی(قصور وتقصیر پزشكی) در بیمارستان های آموزشی دانشگاه علوم پزشکی تبریز در سال1393

Abstract

خطای پزشكی عبارت است از قصور خدمت یا ارتکاب عمل اشتباه در برنامه‌ریزی یا اجرای فرآیندهای درمانی. هدف این مطالعه بررسی دیدگاه انترنها نسبت به خطاهای پزشکی (قصور و تقصیر پزشكی) در سال1393 میباشد.روش کار: 200 نفر از اینترن‌های دانشگاه علوم پزشکی تبریز مورد مطالعه قرار گرفتند. میزان آشنایی آنها در مورد خطاهای پزشکی، میزان بروز خطاهای پزشکی، مهمترین علل بروز، فراوانی بروز آنها در بخش‌های مختلف و راهکارهای پیشنهادی جهت کاهش بروز این خطاها مورد ارزیابی قرار گرفت.یافته‌ها: در مورد میزان آشنایی اینترن‌ها با مفاهیم خطاهای پزشكی 3/90% از پاسخ‌گویان با این مفاهیم آشنایی داشتند و 7/6% با این مفاهیم آشنایی نداشتند. در مورد حیطهی بروز خطاهای پزشكی، 4/27% در حیطه تشخیص، 25% در حیطه درمان دارویی، 1/12% در حیطه نقص در تجهیزات، 5/10% در حیطه درمان جراحی و 5/10% در حیطه شرح‌حال گیری بوده است. در مورد مهمترین علل بروز خطای پزشکی، 8/29% خستگی کادر درمان، 7/17% عدم وجود آموزش‌های لازم، 9/16% عدم دانش علمی کافی و 1/16% عدم مسئولیت پذیری میباشد. بیشترین موارد فراوانی بروز خطاهای پزشكی 5/18% در بخش داخلی، 9/15% در بخش جراحی و 2/13% در بخش اورژانس به ترتیب فراوانی به دست آمد. در مورد مهمترین راه‌حل‌های پیشنهادی جهت جلوگیری و یا كاهش بروز خطاهای پزشكی ؛ 7/34% آموزش علمی و عملی کافی، 9/16% تقویت روحیه کار تیمی و 1/12% پاسخگو نمودن افراد را پیشنهاد كردند., Medical error defined as malpractice or wrong in therapeutic processes. The aim of this study is surveying of intern’s opinion about medical error or malpractice in educational hospital of Tabriz university of medical sciences(2014) Material And Methods:200 interns of Tabriz university of medical sciences and theire knowledge about medical error، the incidence of error ، the most important cause of error ،rate of error in different wards and their offers to reduce the incidence of medical errors ،were evaluated.Results:About the knowledge of intern’s about medical error and its concept; 90.3% of interns were familiar with these consept and 90.3% of interns were not. In the setting of error incidence؛27.4% of error were in diagnosis،25% of error were in medical treatment، 12.1% of error in equipment failure, 10.5% of error in surgical treatment and 10.5% of error in medical history taking. about the most cause of errors، 29.8% of causes related to treatment team’s fatigue، 17.7% of causes related to the lack of education practical skills ، 16.9% of causes related to the lake of sufficient scientific literacy and 16.1% of causes were the lake of responsibility. The rate of incidens of medical error in different wards; internal ward were 18.5%، surgery and emergency wards were 15.9% and 13.2% respectively.About the most important suggestions solution to decreasing the error، 34.7% chose adequate educationing of scientific and practical skills and 16.9% chose strengthening of team working spirite and 12.1% chose the responsibility.
