| dc.description.abstract | کراتوکونژنکتیویت آدنوویروسی نوعی عفونت شایع است که به طور عمده ملتحمه و گاهی قرنیهی چشم را درگیر میکند. درمان‌های حمایتی در EKC شامل، استفاده از اشک مصنوعی، کمپرس سرد، استفاده از عوامل سیکلوپلژیک، کورتیکواستروئید موضعی، تجویز عوامل ضدّ ویروسی موضعی میباشد. بسته به شدت بیماری بیماران باید روز‌ها تا حتی هفته‌ها پیگیری گردند. پوویدون-ید به عنوان ضد عفونی کننده موثر در آماده سازی برای جراحی عمومی، جراحی های چشم، و ضد عفونی کننده آزمایشگاهی استفاده می شود. پوویدون-ید رقیق بعنوان مهار کننده ویروس های متعدد، باکتری ها، قارچ ها، و انگل ها استفاده میگردد. استفاده از این محلول میتواند، از مصرف بی مورد آنتی بیوتیک ها و استروئید ها جلوگیری کند.لذا بر آن شدیم که مطالعه ی حاضر را جهت بررسی اثربخشی محلول پویدون-ید در درمان EKC به انجام برسانیم.روش کار:پژوهش حاضر کار آزمایی بالینی است. جامعه آماری شامل 102 نفر از بیماران مبتلا به EKC میباشد که بیماری آنها توسط Real-time PCR از نظر وجود DNA آدنوویروس بررسی گردیده است. بیماران به دو گروه 51 نفری تقسیم شدند که به گروه اول قطره بتادین 0.4% با فلوکورت هر 4 ساعت به مدت 7 روز و به گروه دوم فلوکورت هر 4 ساعت تجویز گردید. بیماران هر 2 روز از نظر علایم بالینی معاینه و مقایسه شدند. از بیماران خواسته شد که پس از 3روز، هفت روز و دو هفته جهت کنترل مراجعه کنند. تحلیل های آماری با استفاده از SPSS-IBM 23 انجام شد. یافته ها: نتایج درمانی در مورد کراتیت اپیتلیال نقطه‌ای (p= 0.02)، کموزیس (p=0.00)، درد، خارش، سوزش (p=0.01) واشک ریزش (p=0.047)، در گروه یک بهبود بیشتری نسبت به گروه 2 داشتند. در مورد وجود ممبران و سودوممبران (p=1)، خونریزی کونژ (p=0.19)، سرخی چشم (p=0.9)، photophobia (p=0.76) و احساس جسم خارجی (p=0.2)، تفاوت معنی داری بین دو گروه مشاهده نشد.,
epidemic keratoconjunctivitis (EKC) is a common infection which mostly engages conjunctiva and cornea. A previous history of ophthalmic examination for patients or their families is also frequent. Supportive care includes; artificial tears, cold compresses, prescription of topical cycloplegics, corticosteroids and antiviral agents. Depending on the severity of the disease, patients are to be followed for days and weeks. Povidone-iodine is used as a microbicide in general and ophthalmic surgeries, and as an antiseptic in laboratory setting. Diluted povidone-iodine is also known to have antiviral, antibacterial and antifungal effects. It is considered that this compound can also be used in treatment of EKC, limiting the use of antibiotics and corticosteroids and so reducing the preceding side effects. This study surveys the efficacy of povidone-iodine ophthalmic solution as a treatment of EKC. Materials and methods: This clinical trial is consisted of 102 patients whom their disease is diagnosed by Real-Time PCR (Documented as Adenoviral keratoconjunctivitis). Patients were categorized into two treatment groups (each group consists of 51 patients); group one, receiving Povidone-iodine 0.4% and flucort as the treatment regimen and group two, receiving only flucort regimen. Patients were examined every two days after the treatment onset. All statistical analysis is processed using SPSS-IBM 23.Findings: The present study, shows that group one had better outcomes and earlier symptom relief, concerning some symptoms of EKC including punctate epithelial keratitis (p=0.02), Chemosis (p=0.00), pain, itching, burning (p=0.01) and tearing (p=0.047). however, symptoms including; the presence of membrane/Pseudomembrane (p=1), conjunctival hemorrhage (p=0.19), redness (p=0.9), photophobia (p=0.76) and foreign body sensation (p=0.2) were resolved equally in both treatment groups. | |
