مقایسه میزان اصلاح انحراف بینی با دو تکنیک L-strut graft و spreader graft

Abstract

Introduction: A deviated nose is the one which its vertical axis is different from the vertical axis of the face. The correction of this deformity is challenging subject due to functional (airway obstruction) and cosmetic problems. There are some different techniques for correction of the deviation. Spreader grafts and L-Strut grafts are two most useful techniques. The aim of this study is to compare the correction of the septal deviation in two spreader and L-Strut grafts techniques. Methods: In a clinical trial in Tabriz Imam Reza hospital in years 2009 to 2011, 50 patients with nasal septum deviation candidate for rhinoplasty in two equal groups. The patients were enrolled into one of groups randomly after achieving the inclusion criteria and if the exclusion criteria did not meet them. The Spreader graft technique was used for one group and the L-Strut graft was used for other, the results of surgery was recorded before and 6 months after operation and was analyzed and compared in each group and between the two groups. Results: From 50 patients, 33 (66%) were male and 17 (34%) were female. The minimum age of patients was 18 years and the maximum age was 30 years with a mean age to 23.502.77 years. The results of surgery in cases of severe and moderate nose deviation had significant difference between two group and the excellent and good results were more in the L-Strut group (P=0.01 and P=0.04 respectively). In the cases of mild septal deviation, difference between two surgical method was not statistically meaningful (P=0.15)., هدف از این مطالعه مقایسه میزان اصلاح انحراف بینی با استفاده از دو تکنیک‌می باشد. L-strut graft و spreader graftمواد و روش کار:در یک مطالعه کارآزمایی بالینی که در مرکز آموزشی درمانی امام رضا (ع) و بیمارستان‌ذکریا تبریز در فاصله زمانی اسفند ماه 1388 تا شهریور 1390 انجام گرفت، تعداد 50 نفربیمارمبتلا به انحراف سپتوم بینی، کاندید رینوپلاستی را در قالب دو گروه 25 نفری موردمطالعه قرار دادیم. بیماران واجد شرایط به طور تصادفی وارد یکی از گروه ها شدند . برایL-Strut graft و برای گروه دیگر از تکنیک Spreader graft یکی از گروه ها از تکنیک‌استفاده گردید. نتایج جراحی قبل از عمل و 6 ماه بعد از جراحی ثبت و برای هر گروه و بین‌دو گروه مورد تجزیه و تحلیل و مقایسه قرار گرفت.نتایج و یافت هها:از بین 50 بیمار مورد بررسی، 33 نفر ( 66 درصد) مرد و 17 نفر ( 34 درصد) زن بوده اند .کمترین سن در میان بیماران مورد بررسی 18 سال و بیشترین سن 30 سال بوده است که23 سال می باشد. نتایج جراحی در موارد انحراف شدید و /502/ میانگین این مقادیر برابر 77تفاوت معنی داری باهم داشته است L-Strut و Spreader متوسط سپتوم بینی در دو گروه‌استفاده L-Strut به طوری که میزان نتایج عالی و خوب در گروهی که برای آنها از تکنیک‌در افرادی که انحراف سپتوم بینی .(P=0/ و 04 P=0/ شده بود، بیشتر بوده است (به ترتیب 01در آنها، خفیف بود، تفاوت بین دو روش جراحی از نظر آماری معنی دار نبوده.(P=0/ است
