| dc.description.abstract | this study was performed to investigate the effect of heparin in patients with recurrent failure of implantation. In this clinical trial, 100 infertile couples with a history of RIF were randomly divided into 2 groups. Study group (50 cases) taking 5000 IU LMWH subcutaneously daily for 15 days of oocyte puncture in addition to routine luteal phase support after IVF or ICSI procedure, and the control group (50 cases) only received routine luteal phase support. Implantation results were compared in the two groups. 15 days after embryo transfer, HCG in the heparin group was positive in 16 cases (32%) and negative in 34 cases (68%), as well as in the control group, HCG was positive in 15 patients (30%) and was negative in 35 patients (70%). The statistical analysis in terms of heparin effect in pregnancy rate between two groups was not significant with P = 0.5. Endometrial thickness in the heparin group, in 43 patients (86%) was 8-10 mm and in 7 patients (14%) was 10-12 mm, as well as in the control group, in 39 patients (78%) was 8-10 mm and in 11 patients (22%) was 10-12 mm. Statistical analysis showed no significant difference between two groups in terms of endometrial thickness with P = 0.21. Embryos grade in the heparin group, in 31 patients (62%) was A, in 16 patients (32%) was B and in 3 patients (6%) was C, also in the control group, in 34 patients (68%) was A, in 15 patients (30%) was B and in 1 patient (2%) was C. The mean oocytes count in the heparin and control groups was 6.25 2.65 and 7.33 3.2 respectively (P = 0.07). The mean produced embryo count in the heparin and control groups was 3.5 1.6 and 5.3 2.5 respectively (P = 0.02). The mean transferred embryo count in the heparin and control groups was 2.5 1.26 and 2.67 1.63 respectively (P = 0.4). Also the mean gestational sac count in the pregnant women was 1.7 1 in the heparin group and was 2.57 1.61 in the control group (P = 0.3).,
این مطالعه با هدف بررسی اثر هپارین در بیماران دچار RIF بدون سابقه ترومبوفیلی انجام شد. در این مطالعه کارآزمایی بالینی، 100 زوج نازا با سابقه RIF به طور تصادفی در 2 گروه قرار گرفتند. گروه مطالعه (50 مورد) علاوه بر حمایت فاز لوتئال روتین بدنبال عملIVF یا ICSI از روز تخمک گیری روزانه 5000 واحد LMWH زیر جلدی به مدت 15 روز دریافت نموده و در گروه کنترل (50 مورد) فقط حمایت فاز لوتئال انجام شده و هپارین تجویز نگردید. نتایج لانه گزینی در 2 گروه مداخله و شاهد با هم مقایسه شد.15 روز پس از انتقال جنین، نتیجه HCG سرم اندازه گیری شده در گروه مداخله، در 16 بیمار (%32) مثبت و در 34 بیمار (%68) منفی بود، همچنین در گروه شاهد، در 15 بیمار (%30) مثبت و در 35 بیمار (%70) منفی بود. در بررسی آماری از نظر نقش هپارین در میزان بارداری با 5/0 = p تفاوت معنی داری بین دو گروه مشاهده نگردید. ضخامت آندومتر در گروه مداخله، در 43 بیمار (%86) mm 10-8 و در 7 بیمار (%14) mm12-10 ثبت گردید، همچنین در گروه کنترل، در 39 بیمار (%78) mm 10-8 و در 11 بیمار (%22) mm12-10 ثبت گردید.در بررسی آماری از نظر ضخامت آندومتر با 21/0 = p تفاوت معنی داری بین دو گروه مشاهده نگردید. Grade آمبریو در گروه مداخله، در 31 بیمار (%62) A، در 16 بیمار (%32) B و در 3 بیمار (%6) C بود، همچنین Grade آمبریو در گروه کنترل در 34 بیمار (%68) A، در 15 بیمار (%30) B و در 1 بیمار (%2) C بود. میانگین تعداد اووسیت ها در گروه مداخله 65/2 25/6 و در گروه کنترل 2/3 33/7 بود (07/0 = P). میانگین تعداد آمبریوی تولید شده در گروه مداخله 6/1 5/3 و در گروه کنترل 5/2 3/5 بود (02/0 = P). میانگین تعداد آمبریوی انتقالی در گروه مداخله 26/1 5/2 و در گروه کنترل 63/1 67/2 بود (4/0 = P). میانگین تعداد ساک حاملگی در افراد باردار در گروه مداخله 1 7/1 و در گروه کنترل 61/1 57/2 بود (3/0 = P). | |
