میزان پالایش گلومرولی در بیماران مبتلا به تالاسمی ماژور

Abstract

In this study, the nature of the kidney injury and possible pathogenetic factors were investigated. We conducted a cross sectional and included 50 patients with B-thalassemia major with the age ranged 2-29 years, all with the proven diagnosis of B-thalassemia major based on clinical manifestations and electrophoresis. Biochemical indices such as serum levels of creatinine, Hb, and ferritin and GFR were measured. Glomerular filtration rate (GFR) was calculated according to the Schwartz formula. Demographic data in addition to the above were registered in a predesigned data sheet. All data analysis was performed using SPSS 16.0. P<0.05 was considered significant. Of the 50 patients in this study, (64%) were male and 36% were female. The mean age across all patients was 16.7 years. Average of Ferritin, Cr, Fe and Hb were 3465.1(ng/ml), 0/69 (gr/dl), 186.5(mg/ml) and 10/06(mg/dl). The average of diagnosis age of disease was 26.8 month. Patients GFR was in normal range (by means of 132). There were no significant differences in GFR between male and female (P=0.119), however GFR is lower in patients with long disease duration (P=0.028)., در این مطالعه آسیب کلیوی و عوامل پاتوژن احتمالی مورد ارزیابی قرار گرفته است.در این مطالعه مقطعی تعداد 50 بیمار مبتلا به تالاسمی ماژور در محدوده سنی 2 تا 29 سال که در تمامی این بیماران ، بیماری تالاسمی ماژور از تظاهرات بالینی و تشخیص داده شده است، مورد بررسی قرار گرفت. شاخص های آزمایشگاهی مانند سطح کراتینین سرم، هموگلوبین، فریتین، GFR اندازه گیری شد. Glomerular filtration rate (GFR) بر اساس فرمولاسیون شوارتز محاسبه گردید. خصوصیات دموگرافیک در پرسشنامه های از پیش تعیین شده وارد شد. اطلاعات توسط نرم افزار 16-SPSS آنالیز گردید. P کمتر از 05/0 معنی دار تلقی شده است.از بین 50 بیمار مورد مطالعه، 64% پسر و 36% دختر بودند. میانگین سنی بیماران 7/16 سال بود. سطح سرمی فریتین، کراتینین، آهن و هموگلوبین: 1/3465 نانوگرم بر میلی لیتر، 69/0 گرم بر دسی لیتر، 5/186 میلی گرم بر میلی لیتر و 06/10 میلی گرم بر دسی لیتر بود. میانگین سن بیماران زمان تشخیص 8/26 ماه بود. GFR بیماران در سطح نرمال (با میانگین 132) بود. تفاوت معنی داری در GFR بیماران دختر و پسر وجود نداشت (119/0=P). در حالیکه GFR در کودکان با طول مدت بیماری بیشتر، بطور بارزی کمتر بوده است (028/0=P).
